*LPN Nurse Care Coordinator Opportunity
*Looking for an experienced nurse to manage the services of our patients. All qualified candidates must have an LPN or RN certification with at least one year experience.
- Daily interaction with patients via telephone and face to face encounters to provide medical education and additional intervention with chronic diagnosis and new diseases.
- Weekly, monthly and quarterly reporting to internal teams, external, and government partners on practice quality and performance.
- Daily follow up of patient’s hospital admissions and specialty care appointments.
Job qualifications: LPN or RN who is looking to transition out of patient care
- Has confidence working independently
- Positive approach to working in healthcare and its changing landscape
- Ability to be flexible to new projects and sudden changes in workflow
- Case Management experience / extreme interest in case management preferred
- Experience creating, reading, and presenting reports
- Great patient relationship builder
- Willing to work outside of the normal hours occasionally
- Willing to travel to other sites occasionally
- Comfortable speaking with patients on the phone regarding health concerns
- Attention to detail and impeccable “follow-up” on tasks
- Word/ Excel proficient, must be comfortable scanning documents, uploading reports from outside portals, and filing appropriately
- Team oriented
- EMR adept
- Skilled with constructing comprehensive nursing notes
This is full time position working in our Linden office 9:00am 5:30pm
