Description
Under the supervision of a Health Center Administrator, with specialized training in the area of physical assessment, provides skilled and diverse nursing care. Performs physical assessments and makes independent nursing judgments about client's needs and provides treatment under physician-approved protocols. Participates in the operation of health center programs, maintains client and program activity records, makes referrals as indicated, and educates clients on health issues.
Essential Functions
  1. Interviews and collects data to obtain complete medical history.
  1. Performs physical examinations. Reviews data to diagnose and treat clients according to established protocols.
  1. Orders and/or collects appropriate laboratory tests and interprets results. Differentiates normal from abnormal laboratory findings and follows up as appropriate. May perform specific lab tests such as pregnancy test, strep screen, urinalysis, stool for occult blood, hemoglobin, and blood glucose.
  1. Makes diagnostic and therapeutic decisions based upon collected data. Prescribes medications according to written protocols. Provides necessary follow-up care.
  1. Assists providers with physical assessments and procedures as needed. Administers treatments, medications, and injections and performs testing per provider orders and health center protocols.
  1. Triages the majority of calls or visits from health center clients. Determines needs and initiates appropriate intervention.
  1. Provides case management for clients who have complicated health care needs to ensure optimal outcome of care.
  1. Arranges and completes paperwork for specialized testing, medical procedures and specialists referrals. Assists in obtaining insurance authorizations and may act as a resource for health plan benefits.
  1. Acts as a resource to clinic personnel in the health care center setting to ensure optimal client care.

  1. Reviews written prescriptions with client, explaining use, purpose, side effects, dosage, and any special instructions. Instructs clients in the use of home health care equipment. Dispenses sample medications as prescribed. Initiates referral to drug assistance programs as needed.
  1. Assesses the immunization status of clients. In accordance with written protocols may administer immunizations and documents on immunization record.
  1. Works with diverse cultures including non-English speaking client to breakdown barriers to obtaining health care.
  1. Responds to all emergencies in the Health Dept.
  1. Maintains client charts, ensuring proper documentation of clinic visits, diagnosis, treatments and medications. Codes patient diagnosis for accurate billing.
  1. Ensures that client records are kept secure and confidential and maintained consistent with Health Department policies and procedures and HIPAA standards. Educates clients and families on their rights as related to privacy of medical information.
  1. As necessary, performs the duties of a Health Center Nurse.

Additional essential functions in the adult Health/sexually transmitted Infection Clinic.
  1. Performs physical exams and collects specimens for the diagnosis and treatment of sexually transmitted infections. Provides treatment based on Medical protocols.
  1. Acts as a resource and provides supervised training to medical residents in the specialized area of sexually transmitted infections.

Additional essential functions in the Child Health Clinic.
  1. Obtains and documents a life history (medical, social and educational) on new refugee children. Obtains a family medical history.
  1. Reviews the anthropomorphic measurements and screenings done on the day of the examination with family, making referrals as indicated.
  1. Reviews nutritional intake and counsels using the food pyramid.
  1. Reviews vehicle safety and laws especially for refugees. Discusses safety issues according to age of the child.
  1. Discusses issues of personal and health concerns with child/teen/family such as drugs, alcohol, and sexuality.
  1. Assesses and teaches about dental health and assists in obtaining dental care as needed.
  1. Serves as a community pediatric nursing preceptor for MSU Pediatric Nursing students.
Other Functions
26. During a public health emergency, the employee may be required to perform duties similar to but not limited to those in his/her job description.

Employment Qualifications
Education: Graduation from an accredited school of nursing and advanced specialized training in Physical Assessment.
Experience: Two years of nursing experience, preferably in a Pediatric or Family Practice office or Pediatric Hospital setting.
Other Requirements: Possession of a current license to practice as a Registered Nurse in the State of Michigan.
