Job description

We are seeking a Neuroendicrine/Gastric MDC-Sr. Clinical Nurse who will be responsible for coordinating care for assigned patients to promote the achievement of clinical outcomes. Provide direct patient care based on the nursing process. This individual will serve as an integral member of the multidisciplinary team, working closely with the program champion, and a wide-ranging group of specialists and providers. Promote improvements in treatment area processes and cost-effective use of resources. Accountable for own decisions regarding patient care. Identifies impending changes in patient condition and individual performance issues. Will be responsible for maintaining or developing a way to collect, analyze and present data related to the size and scope of the program, and will be knowledgeable about the particular tumor, cancer in general, the program and Johns Hopkins research related to this tumor.
Specific Duties & Responsibilities

  • Coordinating care for assigned patients to promote the achievement of clinical outcomes.
  • Providing direct patient care based on the nursing process.
  • Working collaboratively with a multi-disciplinary health care team to optimize team performance.
  • Promote improvements in unit processes and cost-effective use of resources.
  • Accountable for own decisions regarding patient care
  • Identifies impending changes in patient condition and individual performance issues and communicates changes to responsible/ respective providers.
  • Analyzes patient care data to deliver and evaluate care.
  • Triage new patients to MDC team by review of medical records, phone history and review of scans and ensure accurate and timely appointments.
  • Pend blood tests and radiology to the triage attending, when appropriate.
  • Coordination of care between disciplines (medical oncology, radiation oncology, surgical oncology).
  • Serve as link to research nurses and coordinators for clinical trials.
  • Coordinate with Marketing and Development office for fundraising efforts.
  • Practice nursing per clinical protocols and under the nursing scope of practice for the State of Maryland. May perform case management services to patients for initial intake.
  • Identify barriers to service and work with appropriate individuals to remove them.
  • Interact with faculty and trainees in the management of patients seen in the center, during both clinic session and management outside of clinic.
  • Responsible for managing patient treatment programs. Serve as a clinical resource on the nursing assessment of the complex patient. Serve as an expert assisting others to prioritize and individualize plan of care.
  • Develop and manage patient education sessions and materials.
  • Triage and review incoming phone messages and communicate with doctors or patients as needed.
  • May support case management activities of home care nurses and facilitate communication between family, home care nursing, physician, and patient.
  • Serve as patient advocate.
  • In addition to responsibilities of the Clinical Nurse, provide nursing support for outpatient clinical area.

Minimum Qualifications
  • Individual must be a registered nurse, licensed in the State of Maryland or state where practicing.
  • Bachelor's degree in nursing or related discipline required.
  • Require minimum of two years of related nursing experience.
  • Additional experience may substitute for bachelor's degree and related master's degree may be considered in lieu of experience to the extent permitted by the JHU equivalency formula.

JHU Equivalency Formula: 30 undergraduate degree credits (semester hours) or 18 graduate degree credits may substitute for one year of experience.
