Remarkable ways you will be making a difference...
- Creates and implements systems by which post-discharge services occur. This will include frequency, duration and level of service.
- Creates and implements a system by which data are taken to evaluate efficacy of post-discharge services.
- Works with team to obtain ancillary services for the child, which could include BDDS waivers, IEP support, community opportunities, etc.
- Communicates seamlessly with the NDI clinical team in planning for the child’s transition.
- Develops relationships with school systems, waiver providers, etc.
- Supports the team providing transition services to NDI clients.
- When necessary, will provide services to a child/family to prevent the need for an acute hospitalization.
- Actively participates in and/or oversees data collection and the use of data to improve outcomes, practices and service delivery.
- Completes incident reports, including reporting any suspected exploitation, abuse, or neglect of a client.
- Maintains documentation that is accurate and legible.
- Completes and updates mandatory training and other licensure related items in a timely manner.
- Adheres to all policies and procedures as defined in the Employee Handbook and Damar Operational Policies and Procedures Manual.
- Operates Damar vehicles and/or personal vehicles to transport clients or perform other work-related duties.
What you need to be considered...
- Master’s Degree in a social services field.
At least 2 years of experience with children with severe behavioral needs required.
At least 4 years working with those with autism and/or ID/DD preferred.
Experience with BDDS services preferred.
- BCBA credential preferred.
- Valid Indiana driver's license and good driving record.
- Be clear of any felony convictions, substantiated charges of neglect or other disqualifying criminal offenses.
- Successful completion of pre-employment drug screen and physical.

Physical: Primary functions require sufficient physical ability and mobility to work in an office setting; to stand or sit for prolonged periods of time; to occasionally stoop, bend, kneel, crouch, reach, and twist; to lift, carry, push, and/or pull light to moderate amounts of weight (up to 35lbs); to operate office equipment requiring repetitive hand movement and fine coordination including use of a computer keyboard; to travel to other locations using various modes of private and commercial transportation; and to verbally communicate to exchange information.
