Job Description
The Speech/Language Pathologist evaluates, develops, implements and documents an intervention plan. This position provides intervention, evaluation and documentation of progress toward goals. The SLP develops and documents a discharge plan, completes all documentation required by multiple service sites or facilities, and meets requirements of third party payers. Job Description Evaluates, develops, and implements an intervention plan using methods that are effective and are meaningful to the patient s plan of care through remediation/restoration, compensation/adaptation, disability prevention and health promotion. Provides intervention, evaluation and documentation of goal progress, modifies intervention methods as need, and works with therapy team members to achieve patients goal. Balances schedules at multiple sites daily and adjust schedules quickly to accommodate changes or new patient evaluations as necessary. Meets requirements of third party payers by including appropriate documentation following window periods and other insurance guidelines. Develops and documents a discharge plan when patient has reached goals, achieved maximum benefit from therapy or does not desire to continue service. Provides appropriate follow-up resources, services and training of patient and family members as appropriate. Utilizes a team approach in the delivery and management of services and patient care. Develops and maintain relationships with key facility staff at all sites including, but not limited to, the administrator, director of nursing, MDS coordinator, and dietary manager as well as the restorative nursing staff. Completes all documentation on time and within practice guidelines. Performs pain assessment and takes appropriate action according to standard operating procedure. Performs other duties as assigned or needed to meet the needs of the department/organization. Hours typically are 8-4:30 pm Monday through Friday, with flexibility of start and end times. At this time, no weekend work or holiday is required. Travel is required with car, gas card included.
Shift: M-F, Days-flexible hours but generally 8-5. Would be open to 4-10 hour days as well.
