Job description

A Navigator is an individual who is in recovery from AOD use and/or GMH for an appropriate period of time (greater than or equal to 1 year) and whose primary role is working with individuals dealing with chronic homelessness as their Navigator for housing, services and ongoing support. Navigators hired specifically for COVID-19 Response will be assigned to work on various COVID-19 response projects that are responding to the needs of individuals experiencing homelessness and have tested COVID-19 positive and/or suspect of COVID-19 symptoms and/or exposure, and/or adults and families vulnerable to, but not exhibiting symptoms of COVID-19. The types of COVID-19 response projects with CBI include but are not limited to, transportation/drivers, hotel shelter staff, shelter staff, outreach and street navigation.

Temporary Status Defined: In an effort to respond to COVID-19 CBI has been awarded multiple projects that are defined as temporary funding only to respond to COVID-19. The length of time of the COVID-19 Response projects are undefined therefore leading to a “temporary” hire status. However, the expectation is that once a Navigator II is hired for COVID-19 

and the project ends, CBI would likely seek to retain the staff on future non-COVID-19 response projects. The opportunity for continuing with CBI after the end of COVID-19 response projects would be contingent upon open positive availability and staff persons skill set and performance.

 

A Navigator follows the policies according to the Arizona Administrative Code (AAC), Arizona Department of Health Services/Department of Behavioral Health Services (ADHS/DBHS) Provider Manual, Commission on Accreditation of Rehabilitation Facilities (CARF), Community Bridges Polices and Procedures, and specific for the Hotel/Shelter Project. The Lead Navigator will also carry out other duties assigned by immediate supervisor and/or clinical supervisor. Each area below is defined by specific function as it relates to policy, process, and the governing rules set forth by the Department of Behavioral Health Services (DBHS), Health Plans, Office of Behavioral Health Licensure (OBHL), and the Code of Federal Regulations. Each employee will be held accountable and responsible for fulfilling each of these duties as well as having the capability to carry them out. 

Navigator Responsibilities

  • The Navigator will be responsible for managing participants’ use of the shelter site.
  • The Navigator will complete the program intake for each participant
  • The Navigator will do a safety search of incoming property at intake.
  • The navigator will assist participants with moving in and provide participant orientation on the use of the hotel.
  • The Navigator will conduct observations of each room every hour to check on each participant and assess the condition of their rooms, these room checks will be tracked on a spreadsheet and saved.
  • The Navigator will assess the property, ensure that participants are smoking in designated areas only, and are not loitering in undesignated areas.
  • The Navigator will coordinate with the hotel for room cleaning and any other housing needs.
  • The Navigator will respond to crisis incidents to de-escalate the crisis and connect the participant to appropriate follow up care as needed.
  • The Navigator will identify if the participant is working with an assigned Outreach Navigator to establish a plan to obtain housing and address any other barriers to housing stability. In some cases, the participant may self-report a connection to another case manager or agency. In these cases, the Navigator will contact the agency and attempt to reconnect the participant, if reconnection is possible. If not possible, the Navigator would proceed with CBI’s engagement/enrollment process.
  • The Navigator will assist the participants’ Navigators in developing and implementing plans to transition from the shelters. The Shelter Navigators can complete a VI-SPDAT assessment (if needed) to enroll the participant(s) in the coordinated entry process for housing, begin on-line application processes for benefits, or connect to other needed services such as scheduling medical appointments and transportation. The Shelter Navigators will update the participant’s Outreach Navigator at least weekly to share observations, any areas of concern, and progress in working toward transition.
