Job Description
Asian Community Care Management, Inc. is looking for a care manager (LMSW/MSW/RN) to provide telephonic coordination care for members through assessment and member-centered care planning. This is a 100% remote position.

Requirements:
- Master Social Worker/Registered Nurse
- License Master Social Worker Preferred
- 1 year+ Homecare Experience Preferred
- 1 year+ Case Management Experience Preferred
