Minimum Data Set (Mds) Coordinator

Summary: This is a full-time position with the FSST LTC Facility. The primary purpose of this position is to conduct and coordinate the development and completion of resident assessments in accordance with the requirements of the state and the policies and goals of this facility.

Education and/or experience:

• Must possess, as a minimum, a Nursing Degree from an accredited college or university, or be a graduate of an approved LPN/LVN program.

Qualifications:

• Must submit and pass pre-employment drug and alcohol testing.
• Must submit to background investigation in accordance with Public Law 101-630, Title IV, Indian Child Protection and Family Violence Prevention Act of 1990.

Physical Requirements:

• Must be able to lift, push, pull, and move a minimum of 50 pounds.
