Mentor (BHT)

Job Description - Mentor (BHT or BHPP)

Essential Duties and Responsibilities:

• Responsible for coordinating and providing services with agencies and families to meet the needs of the patient according to the treatment plan
• Manages a case load of 10-15 children
• Maintains compliance with all company policies and procedures
• Coordinates services according to treatment plan
• Attends Behavioral Health scheduled appointments for children enrolled with CCSN including CFT meetings
• Completes monthly reports for members on case load by the 15th of every month
• Provides majority of the services during groups, respite, transportation, school and in-home services
• Assists children to meet their needs in accordance with treatment plan
• Provides direction and follow-up to families and individuals regarding their concerns and issues
• Creates a safe and No Harm environment for the children and their families
• Drives company vehicle as needed or assigned
• Provides information and referral assistance to families
• Advocates for members with appropriate agencies
• Complies with HIPAA guidelines
• Documents all services provided
• Maintains professionalism with all agencies in our community to be a key asset in coordinating outside resources
• Other duties as assigned
