Responsible for providing individual and group services, receiving competency training in order to provide specialized rehabilitative services which may include: skills training, health and wellness, supported housing, supported employment, benefits assistance, community resource referral and linking, in home monitoring, co-occurring substance use services, and/or health navigation or Care Coordination.. May serve as a telemedicine presenter for medical services. Utilizes a recovery oriented approach to assist individuals in reaching their full potential. Is responsible for timely documentation of all services. Working computer skills including MS Word and Bilingual skills, a plus.
Bachelor's degree Health Services or related field. Demonstrates competencies related to psychosocial rehabilitation and/or skills training activities.
Work Hours: 8-5 (Monday-Friday) Flex
Eligible Stipend: On-Call $100/monthly
Interpreter Stipend: $100/monthly
