This position interacts with individuals served, family members (where a release of information is obtained), as well as Lakes Regional personnel across the system of care. Acts as a liaison to the state funded hospitals and community Mental Health centers, and is responsible for assisting with setting-up transition from inpatient services to community living and outpatient supports. Assist individuals served in making a transition back to community living following psychiatric hospitalization. Reviews all admissions accordingly. Under the supervision of the Utilization Manager will assist the UR program through, but not limited to: MBOW reports, UR meetings, Recovery Planning, Peer Review Audits, and Episodes of Care. Ability to interact verbally with individuals served, family members and providers where appropriate. Knowledge of Lakes Regional and Hospital operating procedures.
Requirements:
Bachelor's degree in a qualified human services field including Psychology, Social Work, Sociology, Special Education, Early Childhood Education, Counseling, Human Growth and Development, Rehabilitation, Gerontology, Physician Assistant, Medicine or Nursing. Two Years experience required as a QMHP-CS in a Community Mental Health center required. Must have a valid Texas driver's license and an acceptable driving record, as well as personal automobile liability insurance as required by the state of Texas. Knowledge of available community resources. Working knowledge and/or the ability to learn Electronic Health Records, MBOW, CARE, Excel, Word, PowerPoint, and other computer related programs as required.
