Job Description
The Adult Therapist for the Smooth Way Home Program works under regular direction of supervisor to provide eligibility determination and direct counseling therapeutic intervention services to adults to improve or maintain the client's behavioral health, emotional, and/or adaptive functioning within various social environments, according to the agency mission and philosophy and department goals. Must hold licensed eligible degree according Arizona Board of Behavioral Health Examiners as a Counselor, Marriage and Family Therapist, or Independent Social Worker, or hold a bachelor’s degree in behavioral health related field and have two (2) years behavioral health work experience.
Job Duties:
- Provides counseling services to parents whose babies have recently transitioned home from the NICU
- Facilitates the development and implementation of an individual treatment plan for each enrolled client in collaboration with the client.
- Provides therapeutic intervention appropriate to the client based on the biopsychosocial assessment, diagnosis and treatment plan
- Conducts periodic reviews of the individual treatment plans
- Transitions discharged/transferred clients and develops discharge or aftercare plans
- Maintains accurate and timely documentation of services, including individual/family sessions, staff meetings and required clinical chart elements
- Develops and maintains positive professional relationships with community health, welfare and social agencies, and reaches out to the community to develop new resources to meet participant and clinic/program needs
- Reports suspected or actual child abuse/neglect to the Department of Child Safety Protective Services or local law enforcement agency in accordance with procedures outlined in Clinical Policies
- Demonstrates sensitivity to linguistic, cultural, social, economic, individual and role differences among persons and families served.
- Adheres to the SWHD Core Values, the Codes of Ethics of NASW and NAEYC, and requirements of behavioral health licensing and funding sources.
Qualifications:
- Must hold licensed eligible degree according Arizona Board of Behavioral Health Examiners as a Counselor, Marriage and Family Therapist, or Independent Social Worker.
- Experience in the assessment, diagnosis and treatment of adults.
- Experience with economically disadvantaged and/or culturally diverse populations.
- Fluent in oral and written English is required. Bilingual (English/Spanish) oral and written fluency preferred.
- Certification in infant/toddler first aid and CPR is required.
- Must be at least twenty-one (21) years of age.
Strength and Mobility:
- Ability to lift 35 to 50 pounds
- Ability to get up and down from the floor without assistance
- Ability to climb flights of stairs
