Pathways to Housing DC (Pathways DC) is an innovative and nationally recognized nonprofit organization committed to ending homelessness for individuals with serious mental illnesses, co-occurring substance abuse disorders and/or other medical challenges in our Nations capital. Pathways is one of the originators of the “Housing First” model, in which housing is offered to those in need without pre-conditions.

The Hope Has a Home program is a partnership between Volunteers of America, AmeriHealth Caritas DC (ACDC), Unity Health Care, and Pathways to Housing DC. This program provides short-term residential/respite care that allows ACDC members, including those experiencing homelessness, to rest and recover in a safe environment while accessing medical care and other support services. After four years of operating the male-identified respite program, we are continuing our partnership to open the womens program, to provide specialized services for this population. The Hope Has a Home program is operated in two sites: Phase One: a residence where medical services are provided to support each individual in achieving medical stability, and Phase Two: a residence that primarily focuses on continuing social support services and facilitating the participant to increase their independence around their care. The Medical Respite Clinical Coordinator provides services at both Hope Has Home sites to ensure continuity of care and support the goals towards discharge planning. The Medical Respite Clinical Coordinator will be working in the women’s medical respite site and is responsible for working with program participants to improve outcomes and thereby assist program participants to address social needs, access resources, and move on to more stable housing. The Medical Respite Clinical Coordinator develops treatment plans in collaboration with the interdisciplinary team and participant to address barriers and identify needs, with a goal to support each individual to transition back to the community safely and with housing supports in place.


**This is a full-time position, 40 hours per week, Monday- Friday. Approximately 90% of work is conducted onsite, but some work is required in the community to meet with program referrals and complete case management tasks as needed.**


Responsibilities

Program Administration

● Coordinate and/or conduct orientation/intake meetings for new admissions

● Obtain and prepare required client information for data entry into district-wide Homeless

Management Information System (HMIS) database

● Maintain client records and complete notes on interventions provided to residents

● Coordinate with program partners to facilitate operations of the Hope Has a Home program

● Participate in case conferencing to address client needs/issues within the multidisciplinary team

● Maintain all tracking information on referrals, admissions and discharges to the program

● Provide crisis intervention as needed and maintain reporting standards for any incidents

Case Management

● Conduct assessment of program participant’s current level of self-sufficiency

● Conduct VI-SPDAT and full SPDAT assessments and provide housing navigation assistance

● Assist program participant with developing a service plan, to include goals and objectives, based on needs identified in assessment, while facilitating choice, autonomy, and self-determination

● Meet with program participants to support goals outlined in the service plan

● Coordinate service provision among different providers as needed including connecting program participants to behavioral health and substance use services

● Assist individuals in applying for benefits including SNAP, and Supplemental Security Income/Social Security Disability

● Thoroughly and accurately, document all interventions in the electronic medical record.

● Complete monthly statistics on case management services

● Provide supportive counseling as needed

Advocacy

● Attend Coordinated Entry meetings to advocate on behalf of program participants

● Make follow up contacts to determine program participants status in housing placements, treatment, or other facilities

● Advocate for participants to improve their access to specialized counseling, employment, medical and Mental Health services, transportation and any other needed services

● Maintain current knowledge of community resources and service providers

● Provide crisis-intervention as needed

● Assist with coordination of transportation to medical appointments etc.

● Perform related work as assigned.

Qualifications

Master’s degree in Social Work or Counseling from an accredited university required

● Currently licensed and insured in the District of Columbia as an LICSW or LPC required

● Minimum 3 years of experience in case management or counseling of people with psychiatric disabilities, histories of substance abuse and/or trauma, and individuals experiencing homelessness

● Demonstrated knowledge of housing services, the DC coordinated entry system, Mental Health, and substance use disorders

● Knowledgeable in trauma informed care to address areas such as domestic violence, abuse, victimization/exploitation, etc.

● Knowledgeable in areas of women’s issues, such as pregnancy, female reproductive health, gender inequalities, etc.

● Experience working in a healthcare or medical facility preferred

● Patient, creative, flexible, compassionate, and passion for working with persons with complex needs

● Excellent organizational, communication, writing, and computer skills

● Bilingual English/Spanish a plus
