- Member Medical Assistant Navigator will coordinate medical appointments, including scheduling appointments with healthcare providers and ensuring members have the necessary information and resources to attend.
- Member Medical Assistant Navigator will help members navigate complex medical issues, such as chronic conditions, disabilities, or mental health.
- Member Medical Assistant Navigator will advocate for members to ensure they receive high-quality and coordinated care, such as ensuring they receive the appropriate diagnostic tests, medications, and treatments.
- Member Medical Assistant Navigator will provide support and assistance to members during medical emergencies or hospitalizations, including coordinating with healthcare providers, communicating with family members, and assisting with discharge planning and home care.
- Member Medical Assistant Navigator will be documenting member interactions and outcomes in a member record or database.
- Member Medical Assistant Navigator will work with Care Managers to establish weekly and monthly assessment goals.
- Member Medical Assistant Navigator will be actively participate in Care Management Improvement Work Group.

Requirements: Must be "Registered Medical Assistant" as certificate or diploma.
Compensation:
Pay: $15- $18.00/hour
