Job description

: The Medication History Technician performs medication history assessments for new admissions to the hospital, providing complete information regarding each medication identified (i.e., drug, dose, route, frequency, indication for PRN, and last dose taken-date/time). This individual also verifies patient allergy information and ensures complete documentation (i.e., substance, allergy category, reaction description). The Medication History Technician also coordinates medication history assessment with nursing and provider staff and documents the medication history and allergy information in the electronic medical record accurately and in a timely manner. Cross-training in other technician positions and operational functions is expected to provide quality service throughout the organization.

Job Responsibilities include:

  • - Accurately and efficiently takes medication histories from assigned patients.
  • - Effectively contact appropriate sources of information (PCP, SNF, retail pharmacy, etc.)
  • - Tracks admitted patients when needed.
  • - Proficiently uses Meditech and Dr. First for data entry purposes.
  • - Follows up with nursing and provider staff regarding Medication Reconciliation issues.
  • - Communicates effectively with pharmacy department regarding Medication Reconciliation issues.
  • - Anticipates and responds to patient needs during face-to-face interviews.
  • - Cross-trains in other technician functions to support the operations and services of the Pharmacy Department.
  • - Is flexible in assuming other appropriate responsibilities not noted above.

Requirements:

Education - High School diploma/equivalent.

Experience - 2-3 years retail pharmacy technician experience preferred, but not required. Must be at least 18 years old.

Licensure - National certification (NHA/PTCB) or eligible.

Physical Requirements - Repetitive use of hands and fingers (e.g., use of computer keyboard); may require lifting and carrying light loads (up to 40 lbs.), including boxes, equipment, unit-dose cassettes, and IV solutions and stooping and kneeling (e.g., to pick up items from the floor, to remove and replace items on lower shelves, and to file documents in lower file drawers); sitting, walking, or standing for long periods of time (4-8 hours) is often necessary; must be able physically to operate the equipment used for the job.
