Job Description: We are seeking a detail-oriented and compassionate Medication Reconciliation Specialist to join our team on a part-time basis. As a Medication Reconciliation Specialist, you will play a vital role in ensuring the accuracy and safety of our clients' medication regimens.
Key Responsibilities:
- Conduct interviews with patients to obtain comprehensive medication histories.
- Communicate with pharmacies and physician offices to verify and update medication information.
- Assist pharmacists by serving as a knowledgeable resource in Medication Reconciliation processes.
- Support nursing and therapy teams in accurately transcribing medication orders.

Requirements:
- Active Registered Nurse (RN) license.
- Strong attention to detail and organizational skills.
- Excellent communication and interpersonal abilities.
