Job description

Position Summary:
The Medication History Pharmacy Technician, under the supervision of a licensed pharmacist, assists in the collection and distribution of patient medication histories and corresponding information.  The major job responsibilities of this position are: patient interviews, data collection, accurate documentation, and data monitoring.

Qualifications:

Educations:

  • High School Diploma required.
  • Completion of an accredited certified Pharmacy Technician training program required.

Experience:

  • Minimum of two (2) years of demonstrated experience in retail or hospital pharmacy.

Licensure/Certification/Registration:

  • State Certified Pharmacy Technician (CPhT) registration required.
  • Completed and passed national certification exam (PTCB or ExCPT) required.

Duties & Responsibilities:

  • Identifies the names, dosages and indications for major outpatient drug therapies as well as appropriate means of information retrieval, verification and documentation for safe medication use.
  • Builds rapport and conducts interactions with family in a professional manner using communication techniques that tailored to the appropriate education level; prompts for additional information when needed to provide optimal medication histories.
  • Collaborate with other healthcare providers to ensure that quality and patient’s safety are continuously maintained.
  • Clarifies discrepancies, possible medication omissions, or other questions with supervising pharmacist.
  • Assists with medication assistance programs when needed and appropriate.
  • Identifies medication adherence concerns to the pharmacist to improve patient outcomes.
  • Assists with transitions of care to enhance patient experience and overall quality measures related to patient satisfaction and outcomes.
  • Notifies pharmacist of any patients requesting or requiring medication education.
  • Obtain accurate home medication histories for patients via conversation with the patient, family/caregiver(s) confirming as much as possible with nursing homes, physician office(s) and pharmacies where appropriate for those being admitted optimizing patient care and patient safety.
  • Navigates patient tracking boards and determines patient selection for timely Medication History process.
  • Interacts with pharmacists, nurses, physicians, residents, and interns in relation to timing and completion of patient medication histories and triage of medication histories that need to be completed to facilitate patient disposition.
  • Collects Medication History information in conscientious manner, using credible sources documenting in EHR and place pharmacy progress notes within the EMR to be co-signed by a pharmacist.
  • Uses good clinical judgment to determine the accuracy of Medication History list and solicits other sources of information when appropriate.
  • Inventories and restocks medication in the various dispensing areas as needed.
  • Support the function of the inpatient pharmacy in areas that may include but not limited to: answering telephone calls, unit dose and drug delivery responsibilities.
  • Ensures that medication areas in nursing units are monitored for returns that may be reused prior to expiration and assists the supervising pharmacists with medication management compliance as needed.
  • Stays current on new medication brand/generic names.
  • Demonstrates ability to accurately interpret and follow posted patient isolation precautions.
  • Uses appropriate pharmaceutical medical terms, abbreviations, and symbols common to the prescribing, dispensing, and charting of medication.
  • Keeps area of responsibility clean and orderly.
  • Performs other duties as required or assigned.
  • Participates in the training and/or orientation of new clinical technicians.
  • Maintains confidentiality.
  • Follows all standard safety precautions.
  • Reports on the job as scheduled.
  • Follows the standards of conduct and policies and procedures of Conway Medical Center and applicable laws and regulations and reports violations through the appropriate chain of command.
  • Ability to effectively communicate and work alongside nursing colleagues, physicians, nurse practitioners, pharmacists, financial counselors, and department heads. Strong verbal and written communication skills required.
  • Strong independent critical thinking skills and decision-making skills required.
  • Ability to work on multiple tasks and projects simultaneously required. Must be able to prioritize tasks appropriately, use time management skills effectively, and have problem-solving and critical thinking skills.
  • Must be able to adapt calmly and quickly to changes in workflow, fluctuations in patient census, and accommodate frequent interruptions and urgent requests.
  • Ability to consider and collaborate with providers and nursing staff regarding medication monitoring and history.
  • Interpretation and analysis of detailed information and medication reports.
  • Assist in the development of comprehensive policies and procedures ensuring accurate and reliable pharmacy services.
  • Good listening skills and active participation in interprofessional meetings required.
  • Must be comfortable interacting with patients and family members on a regular basis.
