What you will do:
A Medication Historian conducts patient interviews within the hospital and emergency department to obtain medication lists. A standardized questionnaire and process to obtain a complete medication history is used for each patient. This history is a comprehensive list of prescription medications, infusions, over the counter products, immunizations, dietary and herbal supplements which is entered into the electronic medical record. The Medication Historian technician will use various resources to gather this information including calling physician offices, dialysis centers, pharmacies, hospitals and nursing homes and obtaining medication administration records to confirm outpatient regimens.
Responsibilities:
- Identify the patient to be seen per hospital policy based on alerts and consults
- Complete medication history for each medication identified using an established questionnaire as a guide to obtain a comprehensive list of patient’s entire medication regimen patient's (i.e., medication name, dose, route, frequency, time/date last dose taken). This may involve interviewing the patient, caregiver and or family member.
- Documentation each medication, including prescription, outpatient infusions, over-the-counter medication, dietary and herbal supplements into the electronic medical record. Documents dates and times of last doses administered per protocol.
- Confirm the medication history using a secondary source (i.e., electronic medication history resource, patient’s preferred community pharmacy, MARs, physician office, medication bottles…etc).
- Verify and document patient’s allergies and reported reactions.
What you will need:
- Minimum Education: high school diploma or equivalent.
- Minimum of 1 year pharmacy experience.
- Must become certified pharmacy technician (CPhT) from either PTCB or NHA within 1 year of hire
- Accurate and detail oriented with exemplary time management skills
- Strong communication and leadership skills
- Must be able to write, speak and read English proficiently
- Ability to interact with patients and their caregivers as well as various members of the healthcare team (i.e., nurses, pharmacists, physicians, outpatient providers, nursing home staff)
- Must be willing to interact with patients at the bedside including wearing appropriate PPE and entering patient facing areas
- Knowledge of brand and generic medication names and dosing for the top 200 commonly prescribed medications within 90 days of hire
- Knowledge and skills in using computers and common software applications
- Position requires long periods of standing and significant walking
Pay Range: $20.00 - $26.67
Pay is based on experience, skills, and education. Exempt positions under the Fair Labor Standards Act (FLSA) will be paid within the base salary equivalent of the stated hourly rates. The pay range may also vary within the stated range based on location.
