Job Description:
A Medical Scribe is a trained medical information specialist who focuses on charting physician-patient encounters in real-time during the patient's visit. Scribes work alongside a physician in the outpatient office throughout their day. Medical Scribes are required to follow and observe provider(s) for extended periods Scribes are not patient care providers (i.e. does not touch patients; does not practice medicine). Scribes serves as a liaison between patient and medical staff. A primary goal of a Medical Scribe is to increase the efficiency and the productivity of the physician while complying with the company-specific standards and workflows. Scribes must be able anticipate physician needs to facilitate the flow of clinic.
ESSENTIAL FUNCTIONS:
  • Accompany the healthcare provider into patient examination rooms to transcribe the patient’s care
  • Document using Electronic Health Record and existing templates. Ideally charting is done in real time to assure all information is entered by end of workday
  • Document patient’s history, physical exam, and document accurately the physician’s encounter with the patient and others present including: HPI, ROS, Past Medical and Surgical History, Family and Social histories, Medications and Allergies plus physical examination findings by the physician along with maintaining a problem list within the assessment and plan.
  • During the Exam: enter all proper diagnoses and symptoms as well as follow up instructions and prescriptions as dictated by the physician
  • Enter correct billing codes as dictated by physician
  • Prepare plans for follow-up care, as directed by the provider
  • Transcribe patient orders including laboratory tests, radiology tests, medications excluding chemotherapy/immunotherapy regimens. Document the time and reasoning for patient orders including lab tests, radiology tests and medications.
  • All verbal orders, tasks, emails, texts, etc. for patient care must be communicated by the physician, not the Scribe.
  • Scribes do not participate in any patient care and should refer all requests/questions related to patient care to the responsible physician or nursing staff
  • Document any procedures performed by the physician.
  • Process admitted and discharge paperwork
  • Provide advanced support to providers, including documenting phone calls, alerting the physician with labs/radiology reports are complete, managing faxes, etc.
  • Document the results of laboratory and radiographic studies as dictated by the physician.
  • Document the correct time of patient care related activities, including physician to physician communication, family communication and re-examination of the patient
  • Generate referral letters for physicians as dictated
  • Manage and sort medical documents within the EHR system
  • Assist with e-prescribing of medications as dictated excluding all chemotherapy/ immunotherapy.
  • Transcribe physician dictation to include notes, letters, history and physical reports, hospital admit/discharge/interval notes, sign-out lists, and other physician correspondence or typing as requested. Clarify dictation that is unclear or incomplete, seeking assistance as necessary
  • Apply knowledge of medical terminology, anatomy and physiology to the transcription of proofreading of medical dictation from originators.
  • Ensure physician’s review of all documentation completed by the Scribe, making any necessary amendments, and signing the chart. The physician is ultimately responsible for documentation of the patient’s encounter.
  • Collect, organize and catalog data for physician quality reporting system and other quality improvement efforts and format for submission.
  • Complete data management tasks for physicians to meet documentation/accrediting/insurance agency requirements
  • Chart Prep
    • Identify patient appointments the 2-days prior to ensure if they will be receiving treatment, they are scheduled correctly (work with Infusion Nurses).
    • Ensure patients are following up at the correct times by checking previous progress note
    • Confirm all pending LAB orders are signed the day before the visit to ensure orders can be released by the LAB and timely results can be received during the office visit.
    • Ensure patient charts are pre-charted with any prior testing (radiology or laboratory) results along with orders for blood work to be done on the day of appointment.
  • Assist the physician where need be including: contacting physician’s during the appointment, answering telephone calls and assisting patients.
  • Working with front office and clinical staff to help manage patient care including referrals to other physicians, scheduling radiology appointments and obtaining outside patient information (slides/records)
  • Concentrate on the needs of the provider(s) throughout entire shift.
  • Understand and observe legal concepts (e.g. HIPAA, confidentiality).
  • Support and adhere to Tennessee Oncology policies, procedures, safety and compliance programs as well as the Code of Ethics and human resources and business operations standards.
  • Other duties as assigned by supervisor.
  • Travel to other Tennessee Oncology locations as needed
KNOWLEDGE, SKILLS & ABILITIES:
  • Writing Skills – knowledge and demonstration of proper grammar and spelling in documentation, ability to write concisely, clearly and logically. Exhibit legible hand-writing.
  • Typing Skills – ability to type accurately at 45 wpm + is preferred.
  • Client Service – ability to respond to and anticipate needs, including process constructive critical feedback.
  • Interpersonal Relations – ability to exhibit understanding and respect of others to support and maintain professional relationships.
  • Computer Competency – skilled at operating computers and troubleshooting when necessary.
  • Document Use – ability to use and understand documents required throughout execution of duties.
  • Ethics – personal and professional ethics in observance of legal requirements and company standards.
  • Ability to maintain and adjust to changing needs of provider(s) and to location as a whole.
  • Good judgment, initiative, attention to detail, and the ability to be self-motivated
  • Organizational skills with focus on tracking patient care and improving patient flow.
  • Professional demeanor and recognition of privacy considerations for patients and families.
  • Exhibit excellence in medical terminology and billing & coding knowledge in all documentation.
  • Multi-task efficiently and effectively as required.
  • Work under pressure, within time constraints. Must be able to act calmly and effectively in a busy or stressful situation.
  • Exhibit excellent listening skills. must be discreet, tactful, and modest in performance of duties so as not to distract medical staff from patient care.
EDUCATION & EXPERIENCE:
  • HS Diploma/GED required
  • Training in the healthcare field preferred i.e. Medical Assistant, Medical Transcription/Coding
  • Degrees in the Sciences and Pre-health track preferred. Anatomy, physiology, medical terminology courses are highly desired in individuals with non-science degrees.
