Job Summary
**THIS IS A TELECOMMUTER ROLE FOR THE STATE OF MONTANA**
**Preferred Skillset/experience needed for this position::
Experience with discharge planning within a facility setting.
Knowledge of statewide community resources and transportation resources.
Experience with assisting members/patients with Social Determinants of Health barriers.
Facilitation of provider/primary care follow up.
This position is responsible for working with members on situational issues having direct contact with members and/or families, as appropriate. Will assist the member and/or their family by identifying resource needs, including but not limited to financial, transportation and/or helping with locating support groups and providing clinicians with discharge planning assistance, as applicable. Coordinate referrals for behavioral health services.
JOB REQUIREMENTS:
- Bachelor and/or Master Degree in one of the following Behavioral Health areas: Social Work, Psychology, Human Services
- Minimum of 2 years of experience in health care related field.
- Customer service oriented.
- PC proficiency to include Word, Excel and database experience.
- Excellent verbal and written communication skills.

- Bilingual in English/Spanish or English/Polish.
