JOB TITLE:Medical Liaison
CLASSIFICATION:Full Time, Non-Exempt
PROGRAM/DEPARTMENT: Medical Transport
IMMEDIATE SUPERVISOR: Manager of Medical Transport
POSITION SUMMARY: The Medical Liaison functions as an adjunct member of the Interdisciplinary team, participating in and providing input into the formulation of each individual’s Individual Program Plan (“IPP”). Ensures that the medical component of each IPP is carried out through the coordination of and participation in paperwork, communications and direct services. Promotes individual inclusion, independence, individualization and productivity. Schedules medical/dental appointments and follow-up visits as needed. Accompanies Individuals on medical/dental appointments and assists medical personnel as needed. Advocates for Individuals’ interests. Assist direct care staff with Individuals as needed. Implements Individual Program Plans and documents outcomes as needed. Advocate for individual interests.
QUALIFICATIONS:Minimum Education, Training & Experience:
Ability to perform all job functions
One (1) year experience in Human Services
Preferred Education, Training & Experience:
High School Diploma or equivalent
Two (2) years’ experience in Human Services
ESSENTIAL SKILLS AND ABILITIES:
Valid New York State driver’s license that meets the Agency’s driving policy
Ability and willingness to complete and maintain SCIP-R certification and medication
Ability and willingness to complete medication administration training(s)
Ability to communicate in an effective manner with People We Serve, their families, other staff and outside providers
Be a positive role model for people with disabilities
SALARY:
$16.50 per hour starting rate
$18 per hour after successful completion of required training
