Education
+ High School diploma or equivalent.
Experience
+ One to three years experience in a retail pharmacy setting within the last 3 years
Skills
+ Under the direct supervision of a registered pharmacist by performing a verification of the patient’s medication history and clarification of medication names, doses, frequencies and routes of administration.
+ Must be personable, able to communicate clearly with attention to detail
+ Use of computers is essential, experience with the current hospital information system preferred.
+ Assists in orientation and training of newly hired Medication History Associate.
+ Participates in Medical Center and/or departmental quality assessment and continuous quality improvement activities as directed through identification of quality improvement opportunities, goal setting, collection and evaluation of data, implementation of action plans, or related applications.
+ Performs other related duties as required or requested.
+ Demonstrated ability to read and interpret documents such as safety rules, operating and maintenance instructions and policy and procedures manuals.
+ Knowledge of and trained in age-specific categories being served: neonate, infant, pediatric, adolescent, adult, geriatric.
+ Knowledge of occupational health and safety, ergonomics, workers’ compensation, and OSHA regulations.
+ Demonstrated ability to interpret a variety of instructions furnished in written, oral, diagram, or schedule form.
+ Demonstrated ability to interact with people at various levels in a courteous, tactful and poised manner.
+ Ability to work autonomously to obtain required information for review by a licensed health care provider.
+ Knowledge of medication names, both Trade and Generic
+ Knowledge of non-prescription, herbal and dietary supplement products
+ Knowledge of medical terminology.
+ Able to recognize common medication related adverse reactions upon completion of training
Licensures, Certifications
+ Certified Pharmacy Technician (CPHT) Certification
+ Maryland Registered Pharmacy Technician
Physical Requirements
+ Repetitive use of hands and fingers (i.e., use of computer keyboard). Normal range of hearing and vision to perform work and communicate. Verbal and written communication. Extended periods of standing and walking. Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions.
+ Lifting 20 pounds maximum with frequent lifting and/or carrying of objects weighing up to 10 pounds. Even though the weight lifted may be only a negligible amount, a job is in this category when it requires walking or standing to a significant degree, or when it involves sitting most of the time with a degree of pushing and pulling of arm and/or leg controls.
Working Conditions
+ Fast paced environment, which requires teamwork.
+ Emergency room hospital environment with some exposure to excessive noise, dust, and the like. Occasional exposure to disagreeable temperatures.
Conditions of Employment
+ CPR certification will be required within 90 days of hiring
