Job Summary:
Provide clinical leadership and accountability for the outcomes in the northeast region of Geisinger at Home by participating in an innovative care delivery model and maximizing the use of technology where possible to improve the reliability of care provided and enhance efficiencies. The Medical Director will directly supervise advance practice providers, provide direct patient care in the homes of patients and via telemedicine, provide remote medical care, participate in the education of medical students, and participate in leadership strategies internally and collaboratively with other Population Medicine Initiatives.
Job Duties:
- Provides direct patient care for both Geisinger at Home and Acute Hospital Care at Home in the form of home visits and telemedicine.
- Provides clinical direction and care in a "Remote Medical Command" role on a rotational basis.
- Provides clinical leadership and shared accountability for the performance of the northeast region in partnership with the Nurse Operations Manager.
- Provides direct supervision of advanced practice providers both clinically and administratively.
- Act as a liaison with primary and specialty care providers
- Plays an integral role in Geisinger at Home leadership with a focus on Geisinger's Strategic Priorities to make better health easy: Managing Total Health, Access and Ease of Use, and Performance Excellence.
Clinical work is typically performed in homes across the entire northeast while remote medical care may be performed in both an office-based or home setting. Accountable for satisfying all job specific obligations and complying with all organization policies and procedures.
