JOB SUMMARY: The Medical Case Manager (MCM) will provide Medical Case management services to eligible adult clinic patients at Positive Impact Health Centers, following applicable funding guidelines and best practice care models. The MCM will function as an integral member of an interdisciplinary team which may include the following: medical provider, behavioral health clinician, medication access specialist, clinical pharmacist, and supportive services staff (Community Health Worker, Patient Navigator, Retention/Adherence Manager).
Requirements
- Strong social work, nursing, or case management skills, including the ability to obtain a comprehensive psychosocial assessment and assist adult clients with the development of an individualized service plan.
- Excellent time management and organizational skills.
- Proficient computer skills in word processing, data entry, documentation, and navigation of the Internet.
- Comprehensive and up-to-date knowledge of HIV medical and case management fundamentals preferred.
- Ability to function independently without close supervision.
- Excellent interpersonal skills, and the ability to communicate effectively in a multicultural setting with target populations, volunteers, and agency staff.
- Experience in assessing and developing a care plan with patients.
- Previous professional experience with Electronic Medical Record (EMR) system documentation preferred.
- Excellent verbal and written communication skills are required. Bilingual Spanish is preferred but not required.
- Experience working with individuals from diverse backgrounds. Displays cultural sensitivity in all interactions with patients and staff.
- Experience working as part of a multidisciplinary team preferred.
Minimum Qualifications
- Bilingual English/ Spanish Required
- Bachelor's degree is required but Master's level Social Worker degree (MSW) is a preferred requirement.
- Must have a minimum of one year of experience in a social service delivery system with experience in locating, developing, and coordinating the provision of supportive services in the community.
License/Licensure: Bachelor's degree is required. Master's level Social Worker degree (MSW) is preferred.
