Active driver's license and reliable vehicle.
Ready for an average of 6 patient home visits per day.
Proficient with EMR Systems and tech-savvy.
Available 8am-5pm.

Here's what you'll do:
You'll be at the forefront, visiting an average of 6 homes daily for Annual Wellness Visits.
Verify Primary Care Provider details.
Conduct Medication Reconciliation for accurate medication lists.
Confirm patient allergies.
Administer the Annual Wellness Visit (AWV) Questionnaire.
Record vital signs.
Perform up to 6 Preventive Screenings (PADnet, DPNCheck, Dry Blood Spot Fingerprick test, FIT Kit, Urine Microalbumin Dipstick, DRE exam).
Compensation: $52,000/year - $60,000/year
6-9 visits per day; $40/visit, $20/no show.
