Central Health Care at Home will be a program that is part of a collaborative, multidisciplinary team that will support the clients transitioning from an acute level of care to home. The Care at Home Program will serve as a temporary bridge in providing care to patients until they are transitioned to their primary medical home. This short-term, transitional program is overseen by a physician and provides home-based services using a care team comprised of an advance practice provider, registered nurse, Medical Assistant, and community health worker. Under direction of licensed medical and/or licensed nursing personnel, the MA - Medical Assistant takes a central role in the delivery of health care for the Care at Home Program. This position collaborates with other professional disciplines to ensure effective and efficient client care delivery and the achievement of desired client outcomes. ResponsibilitiesEssential Functions
- Participate in appropriate meetings and trainings, as well as adhere to all company policies and procedures.
- Provide education on MAP benefits and how to access timely care and services
- Scheduling & Training
- Assists with clinical scheduling and provides guidance to team members for assignments and priorities includingtraining.
- Collect, prepare and document specimen collection including, but not limited to: blood, urine, stool, and sputum.
- Provides service to those we are privileged to serve, internal and external, via phone, electronic and in person.
- Applies written and verbal communication skills on multiple levels as well as analysis and critical thinking to support the provider.
- Accurately interview patient and record patient history and measure and record vital signs such as height, weight, BMI, head circumference, current medications, pulse oximetry level and patient comfort level.
- Demonstrate proficiency in clinical information systems.
- Coordinate preventative, wellness, and chronic needs management services using evidenced based practice and care models.
- Coordinate with internal and external social services agencies and programs to ensure that patient receives needed services.
- Complete all direct and indirect care documentation in timely manner and ensure patient records are current and complete.
- Attend daily patient care (unit) huddles and periodic informational (clinic) huddles, meetings, patient conferences, or planning sessions. These may be related to quality assurance, patient care and other related topics within the clinic.
- Participate in quality strategies to evaluate compliance with evidence-based guidelines, standards and to identify opportunities to improve patient outcomes.
- Complete other duties as assigned.
Knowledge, Skills and Abilities
- Demonstrated knowledge of activities associated with the delivery of quality Medical Assistant duties
- High level of skill at building relationships and providing excellent customer service.
- Strong attention to detail and accuracy.
- Ability to utilize computers for data entry and information retrieval.
- Excellent Written and Verbal skills
Minimum Education:
- High School Diploma or equivalent
- Medical Assistant Diploma and/or National Medical Assistant Certification verifying graduation from a Medical Assistant Program or equivalent military healthcare experience.
Minimum Experience:
- 2 years managing populations with complex medical needs
- Completed a Medical Assistant Externship
Licenses and Certifications:
- Certified Medical Assistant (CMA)- AAMA upon hire
- Basic Life Support (BLS) Obtained through American Heart Association or Red Cross upon hire
