Montclair Care Center is seeking an experienced MDS Coordinator to join their team. We are a 64 bed facility nestled in Montclair. We have an awesome team and are looking for an experienced MDS Coordinator to join our team.

· Prior experience in MDS is required

· Experience in MDS/PDPM

· Able to complete assessments and care plans

· Ensure accurate and timely completions

· OBRA calendar

· Monitoring that all departments complete their assessments timely

· Monitoring and auditing clinical records

· Being a team player
