POSITION SUMMARY
This position provides education and case management services to clients who participate in the prenatal care coordination program. This position will assist the Prenatal Care Coordinator to identify and coordinate client identified needs to promote a healthy outcome to pregnancy. Continuation of this position is contingent upon funding allocations.
DUTIES/ RESPONSIBILITIES
Under general supervision, the Maternal-Child Health Worker performs the following essential functions within the framework of the Public Health Core Competencies (PHCC). Adhere to all Tribal Personnel Policies and Procedures and Department Standard Operating Procedures.
Data Analytics and Assessment Skills:
     1. Actively participate in performance management to include quality assurance/improvement (QA/QI) activities.
     2. Assists with coordination of the Prenatal Care Coordinator Program (PNCC).
     3. Provide education and case management for clients as identified by the PNCC.
     4. Assist clients and families with transportation arrangements for PNCC programs and services, including following up on referrals, transport to prenatal appointments at Oneida Health Center (OHC) and the High-Risk Clinic, if referred.
     5. Assist with billing slips, entering Secure Public Health Electronic Record Environment SPHERE data and client service summaries.
     6. Assist with community events and/or educational sessions related to PNCC programming needs. Policy Development and Program Planning Skills
     7. Participate in department strategic planning and strategic direction team activities.
     8. Assist with preparation and presentation of educational materials for PNCC services to individuals and groups.
     9. Provide and maintain appropriate documentation to record client activities, referrals, and interventions.
     10. Support and adhere to established polices and planning endorsed by the Oneida Community Health Center.
Communication Skills
     11. Prepare and provide professional evidence-based Maternal Child Health education including presentations, handouts, displays, and social media content.
     12. Distribute public health education and information regarding services including prenatal and postpartum services throughout the community.
     13. Coordinates the activities of prenatal meetings or events.
     14. Operate business computers and office machines, including in a Windows environment, specifically Word, Excel, presentation software, and electronic medical records.
     15. Operate social media platforms to promote prenatal health and wellness.
     16. Communicate effectively in the English language, both verbally and in writing.
     17. Practice Excellent customer service skills to include but not limited to, addressing customer and employee needs courteously and promptly. Health Equity Skills
     18. Be respectful and show general interest in Native Cultural and social customs and practices.
     19. Develop and provide educational materials and services that include Oneida Nation culture, tradition, values, and are trauma informed.

     20. Develop new relationships and maintain current contacts with local/regional peers, professionals, partners and community organizations for referrals and program collaboration. Public Health Sciences Skills       21. Utilize and apply evidence-based resources and best practices in programming.
Management and Finance Skills
     22. Assist with implementation and monitoring of grant objectives associated with Maternal Child Health programming.
     23. Engage in professional development opportunities relating to Maternal Child Health, Public Health, and trauma informed care.
     24. Actively support the department and Division mission, vision, and strategic plan goals and objectives.
Leadership and Systems Thinking Skills
     25. Ability to work with minimal supervision.
     26. Offer creativity and innovation in addressing and responding to emerging program and client needs.
     27. Due to the need for public health personnel to respond to public health emergencies, the employee must be able to meet the physical demands of performing the following activities:
PHYSICAL REQUIREMENTS/WORK ENVIRONMENT:
     1. Work is generally performed in an office setting, as well as community sites including client’s homes and workplaces.
     2. Work performed in the community is sometimes subject to conditions that range from inclement weather to dangerous conditions such as snow/ ice, cold, heat., Community locations may subject worker to increased risk of driving hazards.
     3. Community locations may subject worker to communicable diseases, insect and other disease vectors, toxins, hazardous materials, chemical and animals. Potential risk of blood borne pathogen exposure.
     4. In all settings, employees may occasionally need to relate to members of the public who exhibit challenging, atypical, or hostile behavior and/ or communication.
     5. Engage in the following movements: balancing, stooping, kneeling, crouching, reaching, standing, walking, lifting, grasping, feeling, and writing and repetitive motions.
     6. Evening and/or weekend work and/or extended hours and irregular shifts may be required.
STANDARD QUALIFICATIONS
1. Basic Life Support (BLS) certification within ninety (90) days.
2. National Incident Management System (NIMS)/Incident Command System (ICS) 100, 200 and NIMS700 certification within three (3) months of employment. National Incident Management System (NIMS)/Incident Command System (ICS) 300 and 400 certification within one (1) year of employment.
3. Complete Oneida Certification on Reporting Child Abuse and Neglect training within ninety (90) days of employment.
4. Employees are required to have proof of immunity or dates of 2 doses of MMR and Varicella prior to starting in any position within the Oneida Comprehensive Health Division. Any refusal of vaccination(s) or failure to provide proof of immunity may disqualify the applicant.
5. Maintain strict departmental security, confidentiality, and quality to meet professional standards of the department.
6. Must adhere to all Tribal Personnel Policies and Procedures, Tribal Standard Operating Procedures, and Area and Program Strategic Plans and Policies.
7. Must be able to be fitted and wear appropriate respiratory protection.
8. Must complete Health Insurance Portability and Accountability Act (HIPAA) training within thirty (30) days of employment and annually thereafter.
9. Must comply with Caregiver Law and Grant conditions criminal history checks as a condition of employment to include fingerprinting.
10. Must be willing and able to obtain additional education and training.
11. Must pass a pre-employment drug screening. Must adhere to the Nation's Drug and Alcohol-Free Workplace Policy during the course of employment.
12. Must pass a background security check with the Oneida Nation to meet the Employment Eligibility Requirements, Tribal/State Compact and/or Oneida Nation Gaming Ordinance as they pertain to the position. A temporary license or Gaming License issued by the Oneida Gaming Commission is required as a condition of employment. and continuing employment within the Oneida Nation Gaming Division.
13. A valid, non-probationary driver’s license or occupational driver’s license, reliable transportation, and insurance are required. Must obtain a Wisconsin non-probationary driver’s license or occupational driver’s license within thirty (30) days of employment if applicant has an out-of-state driver’s license. Must be authorized as eligible to operate a Personal and Tribal vehicle under the Oneida Nation’s Vehicle Driver Certification and Fleet Management Policy prior to actual start date. Must maintain driver’s eligibility as a condition of employment.
PREFERRED QUALIFICATION

2. Previous experience in data collection and/or record keeping.
3. Ability to work independently with a high level of productivity and accuracy.
4. Genuine interest in working with children and female of child-bearing age.
MINIMUM QUALIFICATIONS
