Behavioral Health Recovery Services

Job Scope: Responsible for the management and oversight of Kenneth Young Center’s programming for Front Door Diversion, part of the Williams Consent Decree in Illinois to divert adults with mental illness from placement in a nursing facility. Ensure Front Door grant deliverables are met and complete required tracking and reporting. Coordinate with numerous partners across Northwest Suburbs, plus parts of Lake, Kane and DuPage County. Provide clinical and administrative oversight to Front Door team who provide assessment, community support services, housing, and transition services to persons being diverted from nursing facilities. Oversee development of temporary housing opportunities for individuals served
Primary Responsibilities
  • Oversee overall operations for the Front Door Diversion Program; referrals being diverted from nursing facilities and SMHRF (Specialized Mental Health Rehabilitation Facilities) and approved community-based referrals
  • Monitor the program to ensure 24-hour assessment timeline is being met and effective communication and collaboration occurs with hospitals and state contracted assessors such as PAS and Maximus workers.
  • Ensure that all services and clinical activities provided by all team members are compliant with standards, policies and procedures of the organization and clinical best practices; integrating all facets of clients’ lives including mental/emotional health, physical health/wellness, employment and education, social and family relationships, substance use issues, spiritual, community-integration, and housing needs.
  • Work with FD Supervisors to develop co-location and expand relationships with area hospitals, SMHRF, police departments, MCO insurance coordinators, and homeless service agencies to increase referral sources.
  • Coordinate with FD supervisors and program support staff to complete same-day reports to DMH regarding referral status, when required.
  • Oversee KYC-leased apartments and relationships with area hotels utilized for temporary housing and support development and expansion of temporary housing opportunities in the Northwest Suburbs
  • Monitor, oversee, and approve all temporary housing activities, including client length of stay, client active participation in care and permanent housing search, and behaviors meeting FD program requirements.
  • Monitor and approve all Front Door expenditures to ensure meeting grant requirements, KYC policy, and identify and encourage alternatives to ensure fiscal responsibility.
  • Ensure timely and accurate completion of daily data reporting, weekly, monthly, and quarterly reports with timely submission to DHS/DMH
  • Monitor progress and ensure meeting all grant deliverables and milestone targets for Front Door contract.
  • Coordinate with Manager of ACT/CST, Manager of Supportive Housing, and Manager of Data and Compliance to ensure all team members are working together effectively in care provision and grant reporting.
  • Identify and coordinate client referrals for Housing First No-Income Housing, Employment Services, and SOAR (SSI/SSDI Outreach Access and Recovery) services for FD clients with no/low income.
  • Coordinate communication, service provision, and continue to build relationships with area providers for linkage outside service area.
  • Act as a liaison between agency departments (PSR, SUPR, medication monitoring, Older Adult, Intake, Employment, Supportive Housing, etc.) to ensure service coordination and continuity of care.
  • Monitor program staffing needs and participate in hiring and orientation of new staff, corrective action, and termination as necessary.
Individual Performance
  • Provide same-day consultation to FD assessors to participate in referral decision making.
  • Accurately complete clinical documentation and service reporting in a timely manner meeting KYC policy (currently 5 days for progress notes, 14 days for IM-CANS)
  • Act as primary contact and participant in biweekly FD program meetings, and other scheduled meetings with State Representatives DMH/UIC and hospital administrators
  • Provide backup support and coverage to assessment team and individually complete initial Front Door assessments to ensure 24-hour grant requirement is met based on staffing challenges or influx of referrals; at minimum 2 per month to ensure continued expertise
  • Provide direct service to clients being served by the Front Door program as needed to ensure quality of care and coverage; at least 2-5 visits per week
  • Provide weekly and ad-hoc clinical and administrative supervision of direct reports (Front Door Supervisors, Housing Coordinator, and Program support)
  • Participate in department budget process, assertively participate in monthly finance meetings, monitor progress towards revenue goals and expenditures.
  • Actively monitor all team member’s productivity and grant milestone progress and provide coaching, feedback, and other performance improvement steps as needed to ensure individual and team revenue goals are met.
  • Participation in program planning, service coordination, practice development and quality assurance as a member of PSC (program services committee) and CQI (continuous quality improvement) committee
  • Participate in scheduled department and division manager meetings.
  • Participate in orientations, all staff meetings and training/ development.
  • Interpret and communicate, at the team member and service delivery level, Center policies and procedures.
  • Field calls from clients and FD partners with concerns/complaints and provide effective mediation and customer service.
  • Submit, in a timely and accurate fashion, schedules, timesheets, mileage and other reports required by the Center’s policies and procedures.
  • Review supervisees clinical work including Front Door Assessments, Service Plans, Rule 140 IATP (integrated assessment and treatment plans), LOCUS and DLA, and other comprehensive mental health services.
  • Complete performance evaluations for all supervisees building on strengths and identifying ongoing growth/training needs
  • Participate in clinical/administrative supervision; seek individual consultation with supervisor when needed.
  • Participate in rotating 24/7 backup on-call (licensed staff) and coverage for on-call shifts as needed.
  • Provide in-office support as manager on-site multiple times per week.
  • Other duties as assigned.
Education and Work Experience
  • Masters Degree from an accredited college or university in social work, psychology or a related discipline; meets IL Rule 140 QMHP (Qualified Mental Health Professional) designation
  • Preferred LCPC/LCSW/LMFT in IL that meets Rule 140 LPHA designation
  • Preferred at least 4 years of experience working with individuals with serious mental illness, co-occurring substance use, and/or persons experiencing chronic homelessness
  • Preferred at least 2 years working in non-traditional treatment environments i.e.in-home, residential, hospital, nursing home, community settings
  • Experience in program management and/or development of mental health programming
  • At least 2 years of demonstrated ability to provide clinical and administrative supervision
  • IM-CANS certified or obtain within 30 days
  • Basic technology and computer proficiency: knowledge of Word, Excel, Powerpoint, PDF, Electronic Medical Record systems, typing skills, smartphone messaging and applications, team communication platforms (ex. Microsoft 365) and internet skills
