The Manager of Care Coordination provides operational and clinical oversight of Care Coordinators in housing related case management services to clients. Oversight includes maintaining rigorous processes and procedures to support staff while assisting clients in maintaining independent living, as well as oversight of staff that provides coordinated support services and housing-related counseling. The Manager of Care Coordination will ensure that all required documentation and reports necessary for billing are submitted on time and accurately. The incumbent will understand the importance of client-centered care and needs and provide training that demonstrates and supplements the most effective means of placing the needs and goals of clients first. The Manager of Care Coordination will also supervise Peer Counselors in the Housing & Care Management Department.
- Manages and supports staff; ensures continuity of services when they are absent.
- Provides supervision to staff to ensure effective individual performance and program success, including appropriate training, mentoring and evaluation.
- Develops staff expertise in evidence-based practices, including Housing First, Harm Reduction, Trauma Informed Care, Motivational Interviewing and Relationship-Based Care.
- Ensures high quality of services and coordination with other MDC and community partners.
- Program development and enhancement to ensure the availability of opportunities for tenants to improve stability and well-being.
- Responsible for ensuring continuous person-centered assessment in the area of housing and independent living in the community, strength-based housing care plans, and coordinated care.
- Responsible for oversight of training, performance evaluation, guidance, and recognition.
- Ensures that all aspects of documentation, data entry, and report preparation are timely and accurate via training and oversight.
- Collaborates with Property Management to ensure highly functional integration of services.
- Works actively to integrate Care Management activities with other MDC programs.
- Oversees the management and analysis of data on the clients served by Care Coordinators.
- Utilizes data to manage program outcomes; continuously improving productivity and quality of Care.
- Supports the Director of Housing and Care Management in monitoring the departmental budget.
- Proactively solves problems utilizing innovative practices and a growth-mindset.
- Cultivates and encourages coordinated collaboration within a diverse and multi-disciplinary team.
- Consistently applies a holistic, person-centered approach to client care.

Compensation:
- $ 64K - 67.5K

- Bachelor's degree in Psychology, Sociology, Social Work, or related field required; master's degree preferred.
- Minimum of 5 years' experience working closely with individuals and families in need of assistance, as well as demonstrated knowledge of applying evidence-based practices in Care Coordination.
- Minimum of 2 years' supervisory experience in a relevant field of practice.
- Demonstrated knowledge of incorporating regulations into practices, policies, and procedures.
- Proven experience with Motivational Interviewing, Trauma Informed Care, and Housing First concepts.
- Ability to analyze interventions for effectiveness, research and develop alternative interventions, and openly engage supervisory guidance and input.
- Experience advocating on behalf of individuals with complex needs, such as medical, mental health, and substance use disorders, including individuals with co-occurring disorders.
- Demonstrated knowledge and ability to access entitlement programs, with proven ability to maintain accurate, thorough, timely and complete case files.
- Proven ability to utilize excellent organizational skills to effectively manage deadlines.
- Proficiency with electronic recordkeeping for client information.
- Knowledge of HIPPA, Medicaid, and the WA Administrative Code (WAC).
- Washington State Driver's License and must be insurable for purpose of transporting clients to scheduled appointments and/or visiting and monitoring client's living environment.
