Job description

This position provides support to the Primary Care Service at a VA Medical Center or Health Care System and related Community Based Outpatient Clinics (CBOCs) within the Veterans Integrated Service Network (VISN) 15. The primary responsibility of the position is serving as the principal facility coordinator (PFC) of the Patient Centered Management Module (PCMM) and Newly Enrolled Appointment Requested (NEAR) Coordinator for all Patient Aligned Care Teams (PACT) within Primary Care Service.Qualifications: To qualify for this position, applicants must meet all requirements by the closing date of this announcement, 01/03/2024.
Time-In-Grade Requirement: Applicants who are current Federal employees and have held a GS grade any time in the past 52 weeks must also meet time-in-grade requirements by the closing date of this announcement. For a GS-07 position you must have served 52 weeks at the GS-05. The grade may have been in any occupation, but must have been held in the Federal service. An SF-50 that shows your time-in-grade eligibility must be submitted with your application materials. If the most recent SF-50 has an effective date within the past year, it may not clearly demonstrate you possess one-year time-in-grade, as required by the announcement. In this instance, you must provide an additional SF-50 that clearly demonstrates one-year time-in-grade. Note: Time-In-Grade requirements also apply to former Federal employees applying for reinstatement as well as current employees applying for Veterans Employment Opportunities Act of 1998 (VEOA) appointment.
You may qualify based on your experience and/or education as described below:
GS-07

  • Specialized Experience: You must have one year of specialized experience equivalent to at least the next lower grade GS-05 in the normal line of progression for the occupation in the organization. Examples of specialized experience would typically include, but are not limited to: 1) Experience in work involving the gathering of information; 2) Identifying and analyzing issues; 3) Developing effective recommendations based on; 4) Ability to design and present information to staff or individuals; and 5) Ensure programs are in compliance with policies, guidelines, and business rules. OR,
  • Education: Applicants may substitute education for the required experience. To qualify based on education for this grade level you must have successfully completed one (1) full year of graduate education in a degree program directly related to the work of the position as a substitution for specialized experience. This education must have been obtained in an accredited college or university. Such education must demonstrate the knowledge, skills, and abilities necessary to do the work. In order to be creditable under this provision, the graduate education must have been gained in a curriculum that is qualifying for the position to be filled. As such, course work in fields closely related to those specified include may be accepted if it clearly provides applicants with the background of knowledge and skills necessary for successful job performance. Examples (not all inclusive) of closely related graduate level coursework includes: Business Management, Public Health and/or Healthcare Administration or comparable major of study. OR,
  • Combination: Applicants may also combine education and experience to qualify at this level. You must have equivalent combinations of specialized experience and education described above that meets 100% of the requirements for this position. Equivalent combinations of successfully completed graduate education and specialized experience, as described above, will meet the requirements for this position. If education is used to meet specialized experience requirements, then such education must include courses directly related to the work of the position.
GS-09
Specialized Experience:
You must have one year of specialized experience equivalent to at least the next lower grade GS-07 in the normal line of progression for the occupation in the organization. Examples of specialized experience would typically include, but are not limited to: 1) Experience in work involving the gathering of information; 2) Identifying and analyzing issues; 3) Developing effective recommendations based on; 4) Ability to design and present information to staff or individuals; and 5) Ensure programs are in compliance with policies, guidelines, and business rules. OR,
  • Education: Applicants may substitute education for the required experience. To qualify based on education for this grade level you must have a master's or equivalent graduate degree or two (2) full years of progressively higher-level graduate education leading to such a degree or LL.B. or J.D., if related. Education at the graduate level in an accredited college or university meets the requirements for positions at GS-7 through GS-11. Such education must demonstrate the knowledge, skills, and abilities necessary to do the work. Qualifying coursework includes: Business Management, Public Health and/or Healthcare Administration or comparable major of study.
  • Combination: Applicants may also combine education and experience to qualify at this level. You must have equivalent combinations of specialized experience and education described above that meets 100% of the requirements for this position. Equivalent combinations of successfully completed graduate education and specialized experience, as described above, will meet the requirements for this position. If education is used to meet specialized experience requirements, then such education must include courses directly related to the work of the position.

Experience refers to paid and unpaid experience, including volunteer work done through National Service programs (e.g., Peace Corps, AmeriCorps) and other organizations (e.g., professional; philanthropic; religions; spiritual; community; student; social). Volunteer work helps build critical competencies, knowledge, and skills and can provide valuable training and experience that translates directly to paid employment. You will receive credit for all qualifying experience, including volunteer experience.
Note: A full year of work is considered to be 35-40 hours of work per week. Part-time experience will be credited on the basis of time actually spent in appropriate activities. Applicants wishing to receive credit for such experience must indicate clearly the nature of their duties and responsibilities in each position and the number of hours a week spent in such employment.
Physical Requirements: The work is primarily sedentary with some walking and standing in conjunction with travel to attend meetings and conferences away from the work site. May carry light items such as papers, books, or small computer components. The work does not require any special physical effort.
