Qualifications:

• LVN required
• Advanced First Aid and CPR
• Current TB Test within one year
• Complete understanding of medication and reactions to medication
• Able to monitor vitals
• Ability to work with outside medical support
• Experience working with detox clients
• Knowledge of electronic health record systems
• Daily nursing notes
• Initial nursing medical assessments upon intake

Responsibilities:

• Act as liaison to outside medical professionals and in-house clinical team to order medications and enter doctor’s orders into EHR
• Manage centrally stored medications and medication destruction
• Detox monitoring and documentation (daily vitals, COWS, CIWAS)
• Assist clients with self-administration of medications, schedule outside medical appointments, supervise and work closely with independent medical providers
• Take vitals and note in medication log
• Document all medication reactions and inform Director and physician as required
• Discuss detox protocols and medication reactions with client
• Facilitate life skills groups
• Develop and implement medical trainings as needed for staff
• Complete health pre-screening and assessment, detox treatment plans
• Be an integral part of the case management team
• And, any other duties as assigned by Program Director'
