Lvn Field Nurse- Full Time

Licensed Vocational Nurse (LVN) is responsible for providing direct patient care under the supervision of a Registered Nurse. Responsibilities include following the plan of care, providing treatments, and working collaboratively with the members of the team to help meet positive patient care outcomes.

Nurse to perform home visit in the areas of but not limited to: LA PUENTE, EAST LA, WEST COVINA, EL MONTE, WHITTIER, SANTA MONICA, WEST LA, CULVER CITY, CENTRAL LA, MID-WILSHIRE, PICO-UNION, KOREATOWN, DOWNTOWN, BOYLE HEIGHTS, SOUTH CENTRAL, CRENSHAW, LAX, EL SEGUNDO, HUNGTINTON PARK, DOWNEY, COMPTON, HAWTHRONE, REDONDO BEACH, LOMITA, LONG BEACH, SAN PEDRO, PALOS VERDES.

QUALIFICATIONS:

• Graduate of a state approved school of LVN and currently licensed in CA.
• Minimum of one (1) year experience in nursing, preferred.
• Bilingual: Spanish, Japanese, Mandarin or Cantonese speaking is a plus

RESPONSIBILITIES:

• Provides nursing services, treatments and diagnostic and preventive procedures as assigned.
• Visits patient’s homes for skilled nursing assessments.
• Initiates preventive and rehabilitative nursing procedures as appropriate.
• Observes signs and symptoms and reports to the physician and RN reactions to treatments, including drugs and changes in the patient's physical or emotional condition.
• Teaches and counsels the patient and family/significant others regarding the nursing care needs and other related problems of the patient at home.
• Evaluates with registered nurse the effectiveness of the LPN's/LVN’s nursing service to the patient and family under the guidance of the registered nurse.
• Maintains accurate and complete records of observations, treatments and care of patient.
• Participates in medical record audit as assigned.
• Attends staff meetings, patient care conferences and in services as scheduled.
• Is responsible for participating in patient care conferences to discuss the need for involvement of other members of the health team, such as physical therapist or speech language pathologist.
• Prepares clinical and progress notes.
• Assists the patient in learning appropriate self-care techniques.

Pay: $45.28 - $50.79 per hour
