Definition:
The Licensed Vocational Nurse Case Manager II works within a multi-disciplinary health care team in the delivery of comprehensive services in a community health setting. The Licensed Vocational Nurse Case Manager II assists the client and the health care team in the development and implementation of a health care plan tailored to the individual needs of the client.
Reports To: RN Coordinator or Program Manager of Women’s Health
Qualifications:
1. High school diploma or GED equivalency and satisfactory completion of a state approved program in vocational nursing, plus current license to practice in the State of California.
2. Completion of state approved training and certification (Calif. Business & Professional Code 2069, 2070; and Calif. Code of Regulations 1377, 1377.1) relating to the performance of injections, skin testing, and withdrawal of blood.
3.Must have graduated from an LVN program and have at least one (1) year of experience in an ambulatory care facility with patient education expertise preferred and have worked in one or more of the following areas:
a) Pediatrics – CHDP
b) Family Planning, Title X program
c) Prenatal or Perinatal CPSP
d) Diabetes
e) Adult Care, Chronic Disease
•Experience must have been as a Licensed Vocational or Technical Nurse within a United States Licensing Jurisdiction
•Licensed Vocational Nurse Case Manager II position: three (3) year minimum experience in Maternal and Child Health, CPSP and case management experience, counseling, health education, client advocacy, and community networking.
•Baseline knowledge of Family Planning services and issues required. Completion of the Basic Health Worker Certification within 12 months of hire date and completion of the CPSP training program within 6 months of hire date.
