Job Description
The Licensed Vocational Nurse (LVN) Case Manager will assist and support patients with assessments and care coordination of individuals and families who are unhoused or housing insecure and who may have complex social and medical needs including health care, behavioral health, substance use. The LVN Case Manager will work with patient’s Primary Care team and Specialists as appropriate for an efficient coordination of care to meet the clinical needs of the patient. The LVN Case Manager will anticipate, prepare, and support the patient and/or family members to assist in navigating the health care system including housing resources and facilitate reliable transfers to internal and external care. The LVN Case Manager is accountable for maintaining compliance with current federal, state and local standards, guidelines and regulations, and facility policies. The LVN Case Manager cannot independently evaluate or synthesize data.

Direct & Administrative: Registered Nurse Manager
Professional/Technical: Director of Nursing Services
Qualifications:
1. While an entry level position, experience in practical nursing desirable, preferably in an ambulatory patient facility.
2. Must be a high school graduate or have a General Educational Development (GED) certificate equivalency.
3. Satisfactory completion of a state approved program in vocational nursing, plus current license to practice in the State of California.
4. Must maintain a current Cardio-Pulmonary Resuscitation certification, in compliance with corporate CPR Policy.
5. Fluency in English (speak, read, and write).
6. Fluency in Spanish (speak, translate, read, and write).
7. Must possess excellent interpersonal skills.
8. Must be willing and able to work a varied and flexible schedule to accommodate the needs of NEVHC and its patients.
9. Valid CA Driver’s License, current vehicle liability insurance, and an available vehicle required.
10. Must maintain or exceed competency levels as established by corporate standards for clinical practice.
11. Must demonstrate the knowledge and skills necessary to provide appropriate care.
12. The MIS Patient Information System access level is 2.
