Position Summary:

The LPN, Staff Nurse in the Options Access Center is responsible for providing and coordinating all nursing care for his/her resident based on established clinical nursing practice standards. Collaborates with other professional disciplines to ensure effective and efficient resident care delivery and achievement of desired resident goals. The LPN, Staff Nurse has the responsibility for meeting the established standards of his/her profession and for practicing within the scope of licensure adhering to federal and state regulations. Contributes to the provision of quality nursing care and the demonstration of positive outcomes in resident care.

Responsibilities

  • Administers general nursing care to residents, following the guidance of the Nurse Practitioner/MD/DO. Provides treatments and medications as assigned, adhering to the Standard of Practice for Licensed Practical Nurses in the State of New Jersey.
  • Collaborates with the Nurse Practitioner/MD in planning and coordinating resident care, staff assignments, and supervision. In the absence of the Nurse Practitioner/MD, oversees clinical staff performance, ensuring compliance with policies and procedures. Reports any violations to the Nurse Practitioner/MD.
  • Communicates resident needs or changes in status effectively to practitioners, using the EMR or a similar format for appropriate reporting.
  • Implements physicians' orders with an understanding of their purpose and potential adverse effects. Assists physicians in obtaining accurate medical histories and performing procedures. Ensures accurate and timely documentation for all orders and procedures, including nursing notes.
  • Engages residents in their care plans and conveys residents' needs to the relevant personnel.
  • Records and reports residents' responses to medical and nursing therapy according to established procedures.
  • Consults with Health Care Professionals and nursing staff regarding physicians' recommendations. Monitors lab studies, X-Ray reports, and test results, ensuring timely notification of any significant abnormalities to the physician.
  • Discusses medical test preparations with residents and provides self-help instruction. Offers in-service training for residents' families, identifying areas of need.
  • Contributes to the Interdisciplinary team in developing the Interdisciplinary Service/Care Plan as directed.
  • Directs or assists in staff orientation, teaching new methods of meeting residents' needs, and may assist in nursing care.
  • Investigates and documents incidents such as accidents or medication errors. Reports incidents to the Nurse Practitioner/MD for assessment and follow-up action.
  • Provides care in the community, including apartments and homes.

Education/Experience Requirements

  • Graduate of an accredited School of Nursing
  • Must possess excellent time management, organizational, planning and multitasking skills
  • Must be able to hear, read, write, speak English
  • Must be able to use a calculator, and perform math functions such as addition, subtraction, multiplication, division, percentages for drug calculations
  • Computer literacy

Physical Requirements/Environmental Conditions

  • Must be able to lift up to 35 pounds
  • For up to 85% of the time, must be able to read, write, speak, hear, lift/assist residents into and out of beds, chairs, and the bathroom as well as turn residents in bed and engage in other physical activity, including bending, stooping, carrying, and standing, sitting, or walking for prolonged periods
  • Work in office area(s) as well as throughout the nursing service area (i.e., drug rooms, nurses' stations, resident rooms and homes, etc.)
  • Moves intermittently during working hours. Subject to frequent interruptions.
  • Involved with residents, personnel, visitors, government agencies/personnel, etc., under all conditions and circumstances.
  • Subject to hostile and emotionally upset residents, family members, personnel, and visitors.
  • Communicates with the medical staff, nursing personnel, and other department directors.
  • Works beyond normal working hours, and in other positions temporarily, when necessary.
  • Subject to call back during emergency conditions (e.g., severe weather, evacuation, post-disaster, etc.).
  • Subject to injury from falls, burns from equipment, odors, etc., throughout the workday, as well as to reactions from dust, disinfectants, and other air contaminants.
  • Subject to exposure to infectious waste, disease, conditions, etc., including TB and HIV, MRSA, C-Difficile and Hepatitis B viruses.
  • May be subject to the handling of and exposure to hazardous chemicals.

License/Regulatory Requirements

  • Current, unencumbered State of New Jersey LPN license
  • CPR/AED certified
  • Must obtain all required annual industry related / regulatory immunizations (including completing the annual TB Assessment) and provide documentation of same.
  • Must be free from communicable disease as required by the Department of Health
