Position Summary:
Evaluation of patients at local hospitals for admission to Knollwood on the basis of clinical appropriateness and available resources to deliver a high-quality outcome.
Essential Functions:
- Evaluate patients at area hospitals for admission to the sub-acute unit on the basis of clinical appropriateness and available resources to deliver a high-quality outcome.
- Evaluate patients in terms of contracts held with HMOs such that reimbursement exceeds costs of care.
- Consult with Unit Manager and Therapy Manager to determine appropriate placement.
- Complete all necessary documentation to assist sub-acute unit with admission.
- Represent Knollwood at hospital and HMO meetings related to sub-acute unit policy and procedure.
- Conduct marketing activities with area hospitals and rehabilitation centers to promote business.
You must have strong interpersonal skills as you must communicate effectively with residents, families, visitors, coworkers, physicians and other employees.
Qualifications:
- Minimum -Educational Requirements: RN, LPN or Social Worker (LICSW or LCSW).
- Minimum Work Experience Required: At least one year's experience in long-term care and hospital. geriatric service .as well as interviewing and evaluation experience.
- Physical Requirements:
- Must be able to travel between primary work site and area hospitals· and rehabilitation centers.
- Lifting less than 20 pounds.
- Other Qualifications:
- Ability to plan, organize and work independently.
- Ability to articulate verbally and in writing, the results of evaluations.
- Working knowledge of care implications presented by a wide range of illness and disease entities as well as pharmacy and rehabilitation implications for care and reimbursement.
