Reports To: Clinical Team Leader

Status: Non-exempt (Hourly)

FT, PT, PRN: PRN (as needed shifts)

Shift: Mix of Day and Nights shifts, approximately 8-21 hours per week

Location: In person - Henderson County

Responsible for processing referrals, start of care coordination, nursing triage, and visit support.

Practice within the scope of licenses, certification, or registration, where applicable, and as required by law and regulation (See Attached Components of Nursing Practice for the Licensed Practical Nurse)

• Participating in the assessment of the patient's physical and mental health, including the patient's reaction to illnesses and treatment regimens
• Recording and reporting the results of the nursing assessment
• Participating in implementing the health care plan developed by the registered nurse and/or prescribed by any person authorized by State law to prescribe such a plan, by performing tasks assigned or delegated by and performed under the supervision or under orders or directions of a registered nurse, physician licensed to practice medicine, dentist, or other person authorized by State law to provide the supervision
• Assigning or delegating nursing interventions to other qualified personnel under the supervision of the registered nurse
• Participating in the teaching and counseling of patients as assigned by a registered nurse, physician, or other qualified professional licensed to practice in North Carolina
• Reporting and recording the nursing care rendered and the patient's response to that care
• Maintaining safe and effective nursing care, whether rendered directly or indirectly

• Must be willing and able to care for patients with infectious disease processes unless approved accommodations

• Knowledge of Federal, State and Local regulations and Medicare Conditions of Participation
• Knowledge of HIPAA privacy rules and regulations
• Knowledge of electronic medical records systems

Experience:

• Two years of clinical experience preferred
• Demonstrated skill/expertise in pain and symptom management
• Demonstrated ability to effectively collaborate and facilitate the work of a group
• Demonstrated understanding of reimbursement, regulatory requirements, and Medicare Conditions of Participation

Licensures & Certifications: (if applicable)

• Walking, standing, sitting, bending, reaching, twisting, crouching/squatting, kneeling
• Flexion/Extension of arms/wrist
• Hand dexterity to include gripping, pushing/pulling and writing
• Lifting/carrying of 50 lbs. often
