Job description

Under the direction of the Director of Rural Health Clinics, the Licensed Practical Nurse (LPN) is responsible for assisting in the delivery of health care and patient care management for long-term care patients.  Ensure effective communication about patient care needs with SNFology providers and long-term care facilities.  Primary point of contact with SNFology providers for clinical needs for SNFology patients. 

 

Essential Duties and Responsibilities:

 

  • Provide clinical assistance to providers, patients, and long-term care facilities
  • Process RX refill requests for providers
  • Complete medication prior authorizations
  • Process pharmacy requests for medical equipment (DME), including but not limited to amending chart note, obtaining provider approval/signature, process necessary paperwork (as needed)
  • Complete rounds with provider at nursing homes
  • Perform selected nursing and administrative duties and assist the provider as part of the clinical care team
  • Confer with SNFology providers as needed for daily SNFology patient care needs
  • Confer with provider regarding any incomplete patient tests/consults or other incomplete orders prior to patient visit
  • Coordinate care with disease management or case programs as needed.
  • Assist with scheduling tests and treatments, obtaining pre-certification, if necessary, for hospital care and/or procedures for patients, and process referrals to other providers.
  • Conduct pre-visit planning by reviewing and updating patient charts prior to appointment with recent test results and correspondence and determines needed services per practice protocol
  • Perform administrative duties such as answering phones, scheduling appointments, and maintaining medical records
  • Triage and respond to patient communications within practice policy
  • Complete all necessary documentation in EMR
  • Complete medication reconciliation 
  • Facilitate communication on patient care needs between provider and long-term care facilities.  
  • Review and respond to correspondence for practice/providers
  • Maintain effective communication with SNFology providers, team members and program supervisors
  • Communicate effectively with residents, families, and other healthcare team members
  • Participate in professional development activities and maintain professional affiliations.
  • Create, maintain, copy and file patient files, records and other information as needed
  • Demonstrate consistently a positive attitude and foster teamwork by offering assistance to others.
  • Interact with patients, families, providers, and other staff in a professional, positive, and respectful manner
  • Communicate clearly and effectively with other team members
  • Demonstrates clear ownership of workplace and patient safey
  • Reports mistakes, near misses, adverse events and quality and safety concerns.
  • Participates in the development and implementation of safety and quality improvement activities
  • Protect patient confidentiality and abides by all HIPAA and privacy laws
  • Other duties as may be assigned

 

Preferred but not Required:

  • Knowledge of Microsoft Word and Excel preferred
  • Experiencing using athenaOne EHR system  
  • Knowledge of admissions protocols and procedures within an LTC/SNF environment

 

Job Qualification Requirements:   Current Indiana Licensed Practical Nurse license in good standing required. Experience in long term care or related area preferred.

 

Possession of a valid driver’s license and demonstrated safe driving record.   Reliable transportation required/Partially hybrid position/Travel to nursing home locations and primary office in Worthington, IN required.

 

 

Education: Graduate of an accredited licensed practical nursing program.

 

Licensure/Certification: LPN License, BLS Certification (preferred).  CPR certification required within one month of hire.

 

Experience:  One to three years of clinical experience preferred.  Long term care experience preferred.  Ability to maintain strict confidentiality.

 

Physical Requirements:  Frequent sitting and occasional periods of standing/walking.  Long periods of data entry.  Occasional bending, stooping, and reaching.  Occasionally lift up to 25 lbs.
