Supervisory Related Responsibilities:
1. Demonstrates an ethical approach to leadership, exemplifying the UZRC Core Values.
2. Facilitates the involvement and success of all team members by utilizing individual strengths and providing coaching, performance improvement, coordinating training to promote success.
3. Communicates timely information in the delivery of resident care and all team members.
4. Observes, an applies all policies and procedures.
5. Assures adequate staffing for all shifts of accountability.
6. Ensures a continuity of service delivery with the pre- and post-shifts of his/her shift of accountability.
7. Assures completion of all required departmental job responsibilities and all required documentation.
8. Demonstrates proactive problem-solving skills.
9. Provides timely evaluations for the staff at the end of their initial evaluation timeframe based on hire date.
Position-Specific Responsibilities:
1. Receives report from prior shift’s team leader.
2. Performs narcotics count.
3. Communicates report of residents with Personal Care Resident Aides.
4. Meets the resident’s physical, mental, and emotional needs.
5. Promptly reports and documents resident's significant changes to the attending physician and Primary Power of Attorney (POA).
6. Effectively communicates with all involved in Resident's care.
7. Obtaining/recording vital signs, temperature, blood pressure, pulse, respiration, and pain assessment.
8. Safely administers medications and treatments as prescribed by the physician, assesses potential reactions, and completes required documentation.
9. Assists in developing and implementing Initial and Annual Assessments and Support Plans.
10. Obtains physicians’ orders, removes orders, notifying pharmacy and POA with medication changes.
11. Performs resident’s body and skin assessments.
12. Obtaining and following admission and discharge process.
13. Guides and assists PC Resident Aides with resident care and other assignments.
14. Provides input and assists with PC Resident Aides evaluations.
15. Monitors and tracks lab tests, assists with lab specimen, complete requisitions, and ensure results are forwarded to the ordering physician.
16. Maintains the required minimum 12 hours of annual training related to job duties.
17. Provides residents and families with excellent customer service, striving to exceed their expectations.
18. Completes all necessary reports including 24-hour shift, incidents, med error, DPW as needed, skin assessments, nutrition forms.
Position Qualifications:
1. Must be 18 years of age or older with a high school diploma or high school GED.
2. Must hold a current PA LPN license in good standing or a PA Medication Technician certification.
3. Must have excellent writing, speaking, reading, and comprehension of the English language.
4. Must possess general computer software skills.
Job Type: Part-time
Pay: $18.85 - $22.81 per hour
