LPN or RN Position -
1. Every Other Weekend PM Shift- 2:30pm-10:45pm
2. Casual position- to pick up hours on any shift
LPN Starting- $24.50
RN Starting- $33.25
$1.00/hour shift differential paid on any hours worked on PM & NIGHT shift

Summary: Under the direction of the Director of Nursing, the LPN shall perform professional nursing tasks on her/his own initiative, confer with an RN in-house or on call for assist as needed for decisions requiring professional judgement, or reporting changes in resident's conditions, and perform related work as required.
Summary: Under general supervision of the Golden Age Manor Director of Nursing and Nurse Care Manager, the RN is responsible for total nursing care to all residents on assigned unit during the assigned shift including responsibility for delegation of duties, resident nursing care, staff performance and adherence by staff members to facility policies and procedures.
Duties/Responsibilities
LPN Duties:
Essential Duties: These duties represent the typical duties and responsibilities assigned to this position but may not be inclusive of all tasks performed.
  • Supervising non-professional nursing personnel, and when appropriate, evaluating non-professional personnel.
  • Is designated with the authority to direct all non-professional personnel in his or her charge to act in the interest and welfare of the resident on the unit (to include schedule changes, and job assignments.)
  • Performing regular ordered nursing procedures and tasks including but not limited to: giving medications, irrigating catheters, catheterizing residents as necessary, taking vital signs, sterilizing equipment, etc.
  • Helping nursing assistant with care of resident and caring for resident's with special needs.
  • Assuming charge of a unit during a shift, consulting with an RN as necessary.
  • Making daily rounds of all nursing units under assigned supervision and performing such functions as visiting residents, reviewing of clinical records, medication cards, resident cards, resident care plans and staff assignments, and to the extent possible, accompany physicians when they make rounds.
  • Ability to recognize significant changes in the condition of residents and takes the necessary action.
  • Follows instructions willingly, able to meet deadlines, organizational loyalty, honesty, punctual, and dependable.
  • Adhere to all facility and department safety policies and procedures.
  • Expected to respond to emergency situations involving the safety of residents, other employees and the facility. This includes the ability to assist with a possible evacuation of residents.
Supervisory Responsibilities:
  • Is responsible for the overall direction, coordination, and evaluation as a treatment/medication nurse or charge nurse on the wing/unit they are assigned.
  • Gives timely, appropriate response to resident/nursing staff problems.
  • Channels problems through correct chain of command.
  • Communicates pertinent information regarding residents care to nurses/nursing assistants.
RN Duties:

    • Monitor performance of team members and become involved in staff development, evaluation and discipline when indicated.
  • Planning, developing and implementing resident care plans necessary to the individual's needs.
  • Administering treatments, medications and diagnostic procedures as ordered by the attending practitioner and assigned by administration.
  • Assign nursing care duties to team members and ensure such duties are completed in an appropriate manner.
  • Handling admission transfers and discharges of residents.
  • Basic knowledge of the MDS assessment tool. Including completing quarterly observations/reviews, annual and other required documentation.
  • Assist with pre-admission screening when necessary.
  • Notify resident/appropriate family members of changes to the resident's plan of care or other instances as required by regulation.
  • Ability to recognize significant changes in the condition of residents and to take necessary action.
    • Utilization of AMDA guidelines/federal and state regulation regarding notification of changes to the provider.
  • Expected to respond to emergency situations involving the safety of residents, other employees and the facility.
  • Assisting staff with major nursing care problems, reporting all major problems, accidents and allegations of abuse, neglect, misappropriation and exploitation to the Director of Nursing or Administrator.
  • Making daily rounds to assigned nursing units under assigned supervision and performing such functions as assessing residents, reviewing medications and clinical records, resident care plans and staff assignments.
  • Document nursing care rendered resident response and all other pertinent and necessary data as outlined in facility's policies and procedures
  • Participate in resident and family teaching.
  • Remain current in facility policies, procedures and nursing trends by participating in in-service training programs and continuing education programs.
  • Function as a charge nurse when assigned by serving as a resource person to staff, answer questions, problem solve, and make decisions relevant to resident care.
  • Ability to plan, assign, and supervise the work of others.
  • Ability to establish and maintain effective working and public relationships.

Minimum Skills Assessment Requirements: Must be capable of operating a personal computer using standard word processing, spreadsheet, and e-mail software.
Certificates/Licenses/Registrations: Possession of a current valid Wisconsin Driver's License.

  • Must be able to occasionally push and pull residents weighing more than 75 pounds in wheelchairs and Geri-chairs. Must be able to occasionally reposition residents weighing more than 75 pounds in bed for treatments, nursing procedures and as needed.
  • Must be able to frequently push/pull medication cart requiring force of 10-30 pounds on linoleum and carpeted floors.
  • Must be able to continually perform simple and complex manipulative tasks using resident care equipment, as well as items such as the administration of meds (oral, injection and IV). (Continuously)
  • Must be able to safely assist a resident with transfers and ambulating. Must be able to move about in tight spaces and between objects, equipment, and furniture. (Frequently)
  • Must have hand-eye coordination to administer injections and adjusting flow rates, etc. (Frequently).
  • Constant walking and standing is required. Frequent reaching, bending, kneeling, and squatting are also required to reach lower drawers in Med carts, assist residents, etc.
  • Specific vision abilities required by this job include: Close vision, distance vision, color vision, peripheral vision, depth perception, and ability to adjust focus.
  • Must be able to perform job duties while wearing proper PPE including but not limited to: N95, face shield, gown, glove, etc.
