White Oaks at McHenry is seeking a Licensed Practical Nurse (LPN) to join our team!
2nd Shift (2pm-10pm) opportunity
Monday & Wednesday, every other Tuesday on schedule, no weekends
Our new team member is eligible for a sign on bonus!
Part-time eligible for $750 | Full-time eligible for $1,000
Made payable after 90 days of successful employment
Responsibilities:
- Conducting assigned health assessments, developing service plans, performing health monitoring and personal care programs of the Assisted Living Community.
- Clarifies initial medication orders and contacts the Physician to review the resident's status and medication plans.
- Develops assigned service plans for residents based upon the resident assessment(s). The service plan should be designed to meet the personal care needs of the resident and to monitor significant health conditions, while also promoting resident independence and dignity.
- Promotes resident empowerment and self-care activities including self-medication.
- Performs assigned quarterly and annual reassessments on residents often times utilizing computer and software program. Also performs such assigned reassessments when the resident's health condition warrants.
Qualifications:
- A current state Practical Nurse License (LPN)
- CPR certified
- Exceptional communication and interpersonal skills
- Must demonstrate the ability to serve with love, compassion, and dignity
