The County of Maricopa is looking for a Licensed Practical Nurse who will be working with a team of RN and LPN case managers for our tuberculosis cases. Basic health care services such as preparing patient rooms, performing venipuncture, collecting specimens, and taking patient history to prepare the patients for evaluation will be required. Organization, telephone skills and the ability to educate patients will be required. Collaborative work, including independent and team tasks will be required.
This is a Monday-Friday 8:00am-5:00pm position located in Phoenix, AZ 85006. Up to 5% travel will be required. Must be able to pass a drug screen and background. Hiring multiple positions!
Responsibilities
- The Licensed Practical Nurse for the TB Clinic is in the Public Health Services Division will be working with a team of RN and LPN case managers for our tuberculosis cases. Basic health care services such as preparing patient rooms, performing venipuncture, collecting specimens, and taking patient history to prepare the patients for evaluation will be required. Organization, telephone skills and the ability to educate patients will be required. Collaborative work, including independent and team tasks will be required.
- Assist with management of active and latent tuberculosis cases by reviewing medical records, requesting records, making needed calls to patients and providers and assisting RN and LPN case managers as needed
- Prepares patients for their appointments by accompanying them to the examining room and facilitates the patient flow through the practice of anticipating patient and provider needs.
- Preparing patient charts and ordering labs and chest x-rays
- Ensures exam rooms are clean, stocked and appropriately set up each day prior to rooming patients.
- Performs routine medical tasks of drawing blood, collecting samples, and preparing patients for examinations and treatment.
- Documents patient care using electronic or other medical records systems and computer applications in accordance with regulatory requirements, quality initiatives, policies, and procedures.
- Assists in patient education by providing informational handouts, an after-visit summary, and/or treatment plan summary as instructed by the Clinician.
- Receives and records lab test results via telephone or computer and ensures that those results and related reports are with the patient’s medical record at the time of the patient’s visit.
Minimum Qualifications
- Licensed practical nurse training and certification
- 6 months working as a LPN including venipuncture experience.
- Experience in the use of electronic medical records.
- Certificate of Completion as a Licensed Practical Nurse from an accredited Training Program.
- Vitals, phlebotomy, communication in person and on telephone, electronic health record documentation
- 6 months working as a LPN including venipuncture experience Experience in the use of electronic medical records.
- Certificate of Completion as a Licensed Practical Nurse from an accredited Training Program.
- Two (2) or more years of experience working in the medical field as an LPN including venipuncture experience.
- Bilingual (English/Spanish) skills.
- Case management experience
- Vitals, phlebotomy, case management, patient education, communication in person and on telephone, electronic health record documentation, collaboration with provider and other nurses, problem solving
