LPN (Licensed Practical Nurse) - Float East Hartford and Manchester

First Choice Health Centers (FCHC) is a Federally Qualified Health Center (FQHC) with an exciting, new, LPN position that will float between sites in East Hartford, Manchester and Vernon.

*Annual LPN license renewal reimbursement provided*

• The LPN is primarily responsible for managing urgent and emergent patient calls across all disciplines, in addition to performing licensed professional nursing functions to assess the needs and severity of patients requesting healthcare services.
• The LPN functions as an essential member of the healthcare team, facilitating patient medication refills, managing providers' schedules, counseling patients and other related tasks as needed.
• Vaccine administration experience needed. Can expect to participate at COVID Testing Sites and Flu Clinics in the Greater Hartford area as needed, working closely with the LPN Nursing Supervisor and the Director of Clinical Services to ensure quality care, nursing coverage and patient care throughout the Center.
