Responsibilities: - Provide direct patient care, including administering medications, performing treatments, and monitoring vital signs. - Collaborate with healthcare team members to develop and implement care plans. - Assess patient needs and develop individualized care plans. - Document patient information, including medical history, symptoms, and treatment plans. - Educate patients and their families on healthcare procedures and self-care techniques. - Ensure compliance with all applicable laws, regulations, and policies. Skills: - Pain Management: Ability to assess and manage pain in patients using appropriate interventions. - Nurse: Proficient in providing nursing care and maintaining accurate medical records. - Dementia Care: Experience in caring for patients with dementia or other cognitive impairments. - Cerner: Familiarity with electronic health record systems, specifically Cerner. - ICD-9: Knowledge of the International Classification of Diseases (ICD) coding system. - Hospice Care: Understanding of end-of-life care principles and practices. - IV Insertion: Competency in inserting intravenous catheters for medication administration or fluid replacement. - Inhalation Therapy: Proficiency in administering inhaled medications to patients with respiratory conditions. - Medication Administration: Skilled in safely administering medications via various routes (oral, intravenous, etc.). - Health Coaching: Ability to provide guidance and support to patients in achieving their health goals.
