Job Summary
- Wellness LPNs collaborate with physicians and advanced practice providers to deliver targeted, population appropriate health screenings, and wellness visits.
- Wellness nurses are responsible for assessing the primary care population for gaps in care, and implementing organizationally defined improvement interventions at their assigned clinic(s) with the goal of improving health and wellness of primary care patients.
- Local travel between clinics may be a component of this position.
Qualifications
Education
- High school diploma or equivalent
- Completion of a vocational program for Licensed Practical Nurse.
Experience
- Prefer at least one year experience in a clinical or physician-office setting, emergency room and/or a hospital setting that provides patient care.
- Experience in office clerical activities desirable.
- Knowledge of Medicare covered preventative services, and reimbursement rules helpful.
(Above requirements can be met by equivalent combination of education and experience).
Licensure / Certification
- Current LPN license from the State of Oklahoma is required.
- Basic Life Support (BLS) training or re-training is required during orientation prior to providing patient care.
- Current Basic Life Support (BLS) is required to be maintained for duration of employment.
Compensation/Benefits
- $17.39-$28.40/hr depending on previous work experience.
