Job Summary
Provide direct skilled nursing care to patients/clients in the home setting. Critical multidisciplinary collaboration in all areas including the physician and patient/client/caregiver to reach the goal established in the plan of care. May function in an indirect patient care support role.
Essential Functions
  • Will be expected to do one or more of the following functions:
  • Manage a case load of patients. Make the initial evaluation visit and regularly re-evaluates and assists the patient and caregiver in meeting related nursing needs.
  • Work in office support role; be responsible for a variety of tasks including but not limited to: chart audits, education, infection control, telehealth support, intake and or physician support.
  • Initiate the plan of care and care plan, makes necessary revisions, furnishing those services requiring substantial and specialized nursing skill.
  • Oversee or encompass the liaison between all personnel furnishing services to the patient including LPN's, Home Health Aides and therapists.
  • Initiate appropriate chart documentation preparing clinical and progress notes in a timely manner as indicated in Homecare policy.
  • Demonstrate working knowledge of current regulations and standards. Is prepared for and expected to uphold The Joint Commission standard at all times; may be asked to assist in survey readiness activities.
  • Participate in system wide and staff meetings as needed.

Qualifications
  • Required Associate's Degree

  • 1 year of relevant experience recent clinical experience in home health and/or other clinical setting. Required

  • LIC-Registered Licensed Practical Nurse (LPN) - STATE_MI State of Michigan Upon Hire required
  • LIC-Driver's License - STATE_MI State of Michigan Upon Hire required
