SalaryThis FULL TIME EVENINGLicensed Practical Nurse opportunity is located at our Carolyn's House Hospice Residence and is responsible to evaluate, establish and maintain a comprehensive plan of care that is individualized to specific patient circumstance and offers the patient and family direct clinical care and coordinates the care offered from all interdisciplinary team members.

Our Nurses at our 6 bed Hospice Residence enjoy a 1:6 Nurse to Patient Ratio
Schedule:
Monday through Friday schedule preferred
Qualifications:
- Must have valid PA LPN License. One year Clinical Nursing experience (Hospice or Home Care preferred).
- Excellent communication and interpersonal skills.
- Team oriented
- Strong organizational and multi-tasking skills.
- Knowledge & fluency of Medicare Hospice Benefits and conditions of participation preferred.
Responsibilities (not all inclusive):
- Possesses a knowledge regarding hospice regulations/reimbursement.
- Assess a patient/family’s physical, psychosocial and spiritual needs during each visit.
- Facilitates and coordinates nursing and/or physician orders for the patient.
- Input new patient orders, as well as all patient documentation, into software system in an accurate and timely manner per agency policy.
- Orders supplies and equipment as needed for patient care.
Education:
- Associate (Preferred)
Experience:
- Nursing: 1 year (Preferred)
License/Certification:
- BLS Certification (Required)
- LPN License in PA (Required)
