Description

Organized and assign all jobs to be done on the shift so that the workload is evenly divided among the employees on the basis of staff qualifications, size, and physical layout of the facility, etc.

Qualifications:

  1. Must be a graduate of slate approved school of vocational or professional nursing.
  2. Must have successfully completed state board of registration or licensure and carry a current state license.
  3. Must be willing and able to assume charge of personnel assigned to his/her area.
Responsibilites
  1. Have working knowledge of all residents under his/her care.
  2. Assign all aides to patients spelling out break times, and all special duties to be completed, supervise all aides in performing duties by checking their work closely to be sure the assignments have been completed.
  3. Administer all parental, intramuscular, sub-coetaneous injections.
  4. Make meaningful rounds of all patients utilizing health care plans.
  5. Insures that the individual health care plan is followed. Chart according to policy and procedures, i.e., chart according to MDS. care plan, problems, interventions, and outcome. Directs cha1ting on his/her shifts and makes detailed evaluations of all resident charting so chart reflects progress and condition of resident at all times as according to Pleasant Valley Health Care Center policy and procedure on charting.
  6. Assist in writing and updating health care plan every 30 to 90 days depending on resident's condition and level of care, with the help of the Director of Nursing, Activities Director, and Food Services Supervisor.
  7. Recommend changes needed in staff as to the number of aides and kind of work performed.
  8. Recommend conference with director as needed and participate in staff meetings to keep staff performing at optimum level.
  9. Administer or supervise all treatments prescribed by physician.
  10. Make rounds with physician and record visits in charts.
  11. Transcribe all physicians' orders.
  12. Personally receive or place all calls to the physician.
  13. Administer or supervise the preparation and administrations of prescribed medications
  14. Order all new medications.
  15. Give initial dose of all new medications and chart if medications type warrants close observation.
  16. If medications aide is present, be aware of her/his limitations and do not allow or encourage this limits to be exceeded.
  17. Is responsible for wellbeing and nursing care of all residents assigned to the unit during shift.
  18. Recognize significant changes in the condition of the resident and take necessary action.
  19. Be aware of legal implication if physician's orders are not carried out correctly
  20. Be sensitive to resident's families and respond in an appropriate, professional way, as the situation requires.
  21. Know when the situation cannot be handled and be willing to ask for help and to know whom to ask.
  22. Abide by policies of the facility and ascertain that employees under the supervision of the charge nurse do the same,
  23. Supervise serving of prescribed diets and fluid intake and report persistent unresolved problems to the director of nursing.
  24. Insure that all medications and the person administering the medication or completing the treatment on assigned shifts chart treatments.
  25. Supervise the preparation of incidents and accident reports.
  26. Insure that a copy of medical records or an abstract of them is forwarded with the transfer of residents.
  27. Insure that drugs covered by the Controlled Substance Act are verified by inventory.
  28. Detect and correct situations that have a high probability of causing accidents or injuries to residents.

Job Knowledge:

  1. Knowledge of procedures and techniques necessary to administer medications and treatment as prescribed by physician.
  2. Knowledge of medications and possible side effects.
  3. Knowledge of facility's policies and procedures and how to carry these through.
  4. Knowledge of aging and an awareness of the need for meaningful intervention.

  4. Director of Nursing and/or administration may assign other duties.
  5. Be aware of the Bloodborne Pathogens Exposure Determination Record and listed below are some exposures which you could potentially be exposed: giving injections, administering medications, obtaining laboratory specimens, taking temperatures (oral/rectal) cleansing/dressing wounds, inserting/inspecting tracheal tubes, inserting/inspecting IV's, inserting/inspecting Foley catheters, inserting/inspecting/removing nasogastric tubes, giving emergency first aid, and giving post mortem care.
