What you will do:
- You will provide Case Management and supervisory visits for Adult and Pediatric patients with chronic disabilities in their home. Visits will be for according to patients. Performs on-going case management/supervisory visits according to patient’s needs as ordered by the physician, policy and state regulations to ensure adherence to the plan of care
Who you are:
- Licensed to practice as a registered nurse (RN) by the Board of Registered Nursing in the state of California with a minimum of 1 year Experience in the Health Related Field.
- Training Available
- Spanish Speaking a plus
